1. Objective,Gastric ulcer(GU) occurs when damage factors against gastric mucosa exceedthe defensive and reparative factors. Gastric mucosa' s reparation meansthe function of repair towards harmful factors,such as food, Helicobacterpylori(HP), alcohol, acid, bile salt, non-steroid anti-inflammatorydrug(NSAID), stress and temperature(overheating or supercooling).My tutor——Professor Liu You Zhang created invigorating spleen andbenefiting vital energy Chinese formulated products——Jian Zhong Yu YangPill(JZYYP) by long research of gastric ulcer for several years, clinicalpractice has proved its good curative effect towards tough gastric ulcer.Chinese medicine shows the obvious effect and superiority, and invigoratingspleen and benefiting vital energy is the main therapeutic principle to curedamage of gastric mucosa. The function of Chinese traditional patent drugsto protect gastric mucosahave multiplicate mechanisms. A very active areahas occured about the study of mitochondrion DNA repair enzymes recent years,and the integrity and self-reparation of mitochondrion DNA of gastric mucosais crucial for the recovery gastric ulcer. These research expand our view ofthe damage and self-reparation of gastric mucosa, and we study gastric ulcernot only by external factors,but also consider the balance of externaldamage and self-reparation. But whether there is a relationship betweenChinese drugs and mitochondrion DNA repair enzymes, and how they coordinateeach other,needs further investigation, so our research is to reveal how theinvigorating spleen and benefiting vital energy Chinese drugs regulate themitochondrion DNA repair enzymes, to reveal the mechanism of how Chinese drugscure gastric ulcer, and to find the target of Chinese drugs2. Methods:Our study is to treat gastric ulcer by invigorating spleen and benefiting vital energy Chinese drugs through clinical observation and experimentalresearch, and compared with mucosa protectant——gastropine and H2 receptorantagonist——famotidine about themucosa mitochondrion 8-oxygen guanine DNAglycosylase, thymine glycol DNA glycosylase, 3-methyl adenine DNAglycosylase, the effect of gastralgia, splenic asthenia and quality ofulcer recovery.In clinical research,we observe the rank of gastralgia,splenic asthenia and compositive therapeutic effects. In animal experiment,we detect the body weight, frequency of twisting in unit time, suspending time,index number of gastric ulcer, and microcirculation of gastric mucosa suchas the value of B/(R+G+B),and to clarify the regulation of invigoratingspleen and benefiting vital energy Chinese drugs towards mitochondrion DNArepair enzymes, and the therapeutic effect to gastric ulcer in molecularlevel.3. Results:3.1 Clinical observation:3.1.1 Mitochondrion DNA repair enzymes:8-oxygen guanine DNA glycosylase and thymine glycol DNA glycosylase arelowest in the gastric mucosa of normal group, but they are very high in gastriculcer group,there has extreme difference between them (P<0.01); 8-oxygenguanine DNA glycosylase and thymine glycol DNA glycosylase have no differencebetween gastropine group and gastric ulcer group (P>0.05); 8-oxygen guanineDNA glycosylase and thymine glycol DNA glycosylase in famotidine groupdecreased extremely compared with gastric ulcer group (P<0.01), but stillextremely higher than normal group (P<0.01); 8-oxygen guanine DNA glycosylaseand thymine glycol DNA glycosylase are highest in JianZhongYuYang Pillgroup, and have extreme differences compared with each of the other 4 groups(P<0.01).3.1.2 Grade of gastralgia:Before treatment,there have extreme differences between normal groupand the other 4 groups (P<0.01), there have no differences between the other4 groups (P>0.05).After treatment, grades of gastralgia in JianZhongYuYangPill group and famotidine group are lowest, and has no difference betweenthem (P>0.05); the grade of gastralgia in gastropine group are lower,therehas extreme difference compared with gastric ulcer group (P<0.01), but alsohave extreme differences compared with JianZhongYuYang Pill group and have extremely significant difference compared with normal group (P<0.01),while after treatment, body weight and suspending time in famotidinegroup, gastropine group and model group have no differences (P>0.05), bodyweight and suspending time in JianZhongYuYang Pill group has no differencecompared with normal group (P>0.05),but have extremely significantdifferences compared with model group (P<0.01).3.2.3 Quality of ulcer recovery:3.2.3.1 Index number of gastric ulcer is the direct indication of gastric ulcerhealing. From the results, index number of gastric ulcer in model group isthe biggest, there has extremely significant differences between it and thenormal group (P<0.01), after using drugs, index number of gastric ulcer ineach group has decreased, but still has extremely significant differencecompared with normal group (P<0.01); JianZhongYuYang Pill group and famotidinegroup have obvious differences compared with gastropine group (P<0.05), butJianZhongYuYang Pill group has no difference compared with famotidine group(P>0.05).3.2.3.2 Microcirculation of gastric mucosa such as B/(R+G+B) is the indirectindication of gastric ulcer healing. From the results, the value of B/(R+G+B)in model group has extremely significant difference compared with normal group(P<0.01); after using drugs, the value of B/(R+G+B) in in each group hasincreased, gastropine group has no difference compared with famotidine group(P>0.05), but both of them have extremely significant differences comparedwith normal group (P<0.01); while the value of B/(R+G+B) in JianZhongYuYangPill group has no difference compared with normal group (P>0.05),but haveextremely significant differences compared with gastropine group andfamotidine group (P<0.01).3.2.4 Mitochondrion DNA repair enzymes:3-methyl adenine DNA glycosylase are lowest in the gastric mucosa of normalgroup, but they are very high in gastric ulcer group,there has extremedifference between them (P<0.01); 3-methyl adenine DNA glycosylase have nodifference between gastropine group and gastric ulcer group (P>0.05); 3-methyladenine DNA glycosylase in famotidine group decreased extremelycompared with gastric ulcer group (P<0.01),but still extremely higher thannormal group (P<0.01); 3-methyl adenine DNA glycosylase are highest inJianZhongYuYang Pill group,and have extreme differences compared with each famotidine group. In the difference of grade of gastralgia before and aftertreatment, JianZhongYuYang Pill group has no difference compared withfamotidine group (P>0.05), but gastropine group has obvious differencecompared with JianZhongYuYang Pill group (P<0.05), and has extreme differencescompared with famotidine group (P<0.01).3.1.3 Syndrome of splenic asthenia:Before treatment,the rank of splenic asthenia in each group are balancedexcept normal group. After treatment, the change of splenic asthenia inJianZhongYuYang Pill group are biggest,and has extremely significantdifferences to the other 4 groups (P<0.01), famotidine group and gastropinegroup have extremely significant differences compared with gastric ulcer group(P<0.01), but famotidine group has no difference with gastropine group(P>0.05).3.1.4 Clinical curative effect:Clinical curative effects combined the therapeutic effects towardsgastralgia and splenic asthenia, and give a compositive evaluation ofdifferent drugs. It shows that: JianZhongYuYang Pill group, famotidine groupand gastropine group have extremely significant differences compared withgastric ulcer group (P<0.01), gastropine group has obvious difference comparedwith JianZhongYuYang Pill group (P<0.05), JianZhongYuYang Pill group has nodifference compared with famotidine group (P>0.05).3.2 The results of animal experiment:3.2.1 The results of cure gastralgia:Frequency of twisting in unit time is used to measure the level of gastralgia.Frequency of twisting in model group is the most, there has extremelysignificant difference compared with normal group (P<0.01), while aftertreatment, frequency of twisting in each group has decreased. JianZhongYuYangPill group, famotidine group and gastropine group have extremely significantdifferences compared with model group (P<0.01), JianZhongYuYang Pill groupand famotidine group have obvious differences compared with gastropine group(P<0.05 ), JianZhongYuYang Pill group has no difference compared withfamotidine group (P>0.05).3.2.2 The results of cure syndrome of splenic astheniaBody weight and suspending time describe the changes of splenic asthenia.Body weight and suspending time in model group have decreased apparently, and of the other 4 groups (P<0.01).4. Conclution:4.1 In treatment of gastralgia, both of JianZhongYuYang Pill, famotidine andgastropine have obvious effects. Curative effects of JianZhongYuYang Pill andfamotidine are mainly same, both of them are perfect,and better thangastropine.4.2 In treatment of splenic asthenia, the curative effects of famotidine andgastropine are poor.,have no function of invigorating spleen and benefitingvital energy in principle, but JianZhongYuYang Pill has the best effect tocure splenic asthenia.4.3 In quality of ulcer recovery, curative effects of JianZhongYuYang Pilland famotidine are the mainly same, both of them are better than gastropine.The effect to improve microcirculation of gastric mucosa by JianZhongYuYang Pillis the best, and the microcirculation of gastric mucosa has recoverd to normal. Thetherapeutic effect of JianZhongYuYang Pill is better than famotidine and gastropinestrikingly.4.4 In clinical curative effect, both of JianZhongYuYang Pill, famotidine andgastropine can cure gastric ulcer definitely, compositive therapeutic effectsof JianZhongYuYang Pill and famotidine are the mainly same, butJianZhongYuYang Pill is better than gastropine.4.5 In mechanism of action, because normal gastric mucosa is not damaged, sothe mitochondrion DNA in gastric mucosa is not self repaired, but when thegastric mucosa of gastric ulcer has been damaged, the mitochondrion DNA repairenzymes will repair the damaged DNA in mitochondrion; the mechanism ofgastropine is to give physical protection to gastric mucosa, and has littlerelationship to mitochondrion DNA repair enzymes; famotidine can inhibitsecretion of gastric acid, reduce harmful factors, so the mitochondrion DNArepair enzymes decrease by feedback; but JianZhongYuYang Pill can increasethe mitochondrion DNA repair enzymes greatly to repair the damaged gastricmucosa, and promote the self-cure of gastric ulcer.4.6 Compositive evaluation: to treat gastric ulcer, JianZhongYuYang Pill hasexcellent effect to relieve pain and to eliminate splenic asthenia, it has thesame effect with famotidine to alleviate pain, but better than western drugsin eliminating splenic asthenia and improving microcirculation of gastricmucosa. In mechanism of action, JianZhongYuYang Pill can increase the mitochondrion DNA repair enzymes greatly to repair the damaged gastric mucosa,and promote the self-cure of gastric ulcer, this is the difference to westernmedicine. Western medicine emphasize the counteraction against harmfulfactors, while JianZhongYuYang Pill focuses on self-repair, it is consistentwith the theory of traditional Chinese medicine "supporting healthy energyto eliminate evils", "if healthy energy is inside, evil factors can not comein". So our research give the evidence to show the superiority of Chinesedrugs. We know, the function of Chinese traditional patent drugs havemultiplicate mechanisms to cure disease, our study proved one of the targetsof Chinese traditional patent drugs how to cure gastric ulcer only, that is:Chinese traditional patent drugs can adjust mitochondrion DNA repairenzymes. With the development of molecular biology, we wish we can find moremechanisms of Chinese traditional patent drugs trough experiments in thefuture, and make greater contributions to the cause of investigating,developing the precious wealth——traditional Chinese medicine.
