Gastric carcinoma is one common malignant tumors, lymph node metastasis is most cause of reoccur and bad prognosis. The mechanism of Gastric carcinoma occurring and lymph node metastasis is still unclear. Some oncogene, anti-oncogene and growth factors play important role in Gastric carcinoma growth and proliferation.PTEN is an anti-oncogene, PTEN protein suppress cell growth and induce apoptosis, inhibit tumor metastasis and invasion. IGF-1 and IGF-1R are important growth factor, promote cell growth and inhibit apoptosis. It have been reported that PTEN, IGF-1, IGF-1R involved in occurrence of Gastric carcinoma, but it is still unclear whether all of these three factors involved in occurrence and lymph node metastasis of gastric carcinoma, and their effect on prognosis is disputed.In the progress of tumor development, it shows heterology, and formed different subgroup, promote metastasis. The incidence rate and mortality rate of gastric carcinoma is very high, and one reason is metastasis. Research of gastric carcinoma heterology and PTEN, IGF-1, IGF-1R is rare.Recent work shows VEGFR-1+ bone marrow-derived hematopoietic progenitor cells involve in tumor metastasis, it locate to the pre-metastasis site and formed"niche"suitable for tumor cell cohere and growth. This research has been verified in lung cancer and melanoma, and concluded that all cell express CXCR4 can metastasis via this way. Gastric carcinoma also express CXCR4, it may extent by this way.The study investigate expression of PTEN,IGF-1,IGF-1R in gastric cancer tissue and regional lymph node by immunohistochemistry and reverse-transcription polymerse chain reaction. Expression of PTEN in gastric mucosa was strong, expression of IGF-1, IGF-1R was weak; expression of PTEN was weak in tumor, expression of IGF-1 and IGF-1R was strong. There was significant difference of PTEN IGF-1 IGF-1R positive expression between gastric carcinoma (40.5%,60.8%,63.5%) and gastric mucosa(75.0%,30.0%,35.0%) (P<0.01or P<0.05).2 Relationship between PTEN , IGF-1 , IGF-1R expression and clinicobiological characteristic in gastric carcinoma. 2.1 Relationship between PTEN expression and clinicobiological characteristic in gastric carcinoma. In gastric carcinoma IGF-1 expression was higher in advanced group(65.6%) than early gastric carcinoma group(30.0%)(P<0.05),higher in poor differentiation group(84.6%)than moderate and well differentiation group(47.9%)(P<0.01);higher in lymph node metastasis group(78.9%) than without lymph node metastasis group(41.7%)(P<0.01)higher in stageâ… -â…¡group (39.1%) than stageâ…¢-â…£group (70.6%)(P<0.05).There was no significant difference with age ,gender, tumor location, size(P>0.05). 2.3 Relationship between IGF-1R expression and clinicobiological characteristic in gastric carcinoma. In gastric carcinoma,IGF-1R expression was higher in advanced group(67.2%) than early gastric carcinoma group(40.0%)(P<0.05),higher in poor differentiation group(80.8%)than moderate and well differentiation group(54.2%)(P<0.05);higher in lymph node metastasis group(84.2%) than without lymph node metastasis group(41.7%)(P<0.01), higher in stageâ… -â…¡group (39.1%) than stageâ…¢-â…£group(74.5%)(P<0.01). VEGFR-1 positive express rate in lymph node metastasis is 56.7%3 Expression of VEGFR-1 in liver metastasis Expression of VEGFR-1 in lymph node metastasis is strong.Conclusion:1 Expression of VEGFR-1 in gastric carcinoma may promote cancer progression.2 Expression of VEGFR-1 in regional lymph node metastasis and liver metastasis increased.3 Expression of VEGFR-1 in stem cell around tumor metastasis indicted that VEGFR-1 may mediate tumor metastasis.
