The Relationship Between Untraditional Cardiovascular Risk Factors And Metabolic Syndrome: A Cross-sectional Study In Beijing Community Population

Background Metabolic syndrome (MetS) is a clustering of cardiovascular disease(CVD) risk factors,it has become a large public health issue worldwide. Identifyinghigh-risk untraditional cardiovascular risk factors for incidence of MetS may leadto better understanding of the pathophysiology of MetS.Methods A cohort of1680population in a community-based population inBeijing was investigated. MetS was defined by IDF definition.Multivariate logisticregression analysis was used to estimate the odds ratios(OR) of serum high sensitivityC-reactive protein(Hs-CRP), homocysteine(Hcy), N-terminal pro-brain natriureticpeptide(NT-proBNP),uric acid(UA) for MetS. Multiple linear regression analysis wasperformed to analyze the association between untraditional cardiovascular riskfactors and characteristic variables.Receiver operating characteristic (ROC) curvewas performed to evaluate the usefulness of untraditonal factors for diagnosing MetS.Results Serum Hs-CRP, Hcy, UA were higher in MetS population compared tothose without MetS (p <0.01). The multiple logistic regression analysis revealedthat the higher Hs-CRP, Hcy, UA levels were significantly associated with MetS afteradjusting for confounded factors. Compared with the first quartile,the adjusted ORof having MetS in the second,the third and the forth quartiles were1.378(95%CI:1.012-1.876,p<0.05),1.875(95%CI:1.382-2.545,p<0.01),2.318(95%CI:1.594-2.904,p<0.01) for Hs-CRP, and1.721(95%CI:1.208-2.451,p=0.003),2.237(95%CI:1.562-3.204,p=0.000),4.559(95%CI:3.090-6.727, p=0.000) forUA,repectively. Compared with the first quartile of Hcy, the adjusted OR of havingMetS for the forth quartile was1.379(95%CI:1.005-1.892, p=0.046) after adjustingfor confounded factors.Serum NT-proBNP was lower in MetS population compared tothose without MetS (p <0.01). The multiple logistic regression analysis revealed that NT-proBNP was negatively associated with MetS after adjusting for confoundedfactors. Compared with the first quartile,the adjusted OR of having MetS in thesecond,the third and the forth quartiles were0.710(95%CI:0.497-1.013,p>0.05),0.659(95%CI:0.458-0.947, p<0.05),0.577(95%CI:0.392-0.851,p<0.01),respectively.In multiple linear regression analysis, Hs-CRP was positively related to age,BMI,waist circumference,and negatively related to HDL-c, eGFR. Hcy was positivelyrelated to age,male, BMI, LDL-C, creatinine,uric acid. UA was positively related towaist circumference,BMI, triglycerides, LDL-C, and negatively related tofemale,fasting glucose,eGFR. NT-proBNP was positively related to female, age,systolic blood pressure,and negatively related to waist circumference,diastolic bloodpressure,triglycerides,total cholesterol,eGFR.The optimal cutoff point of hs-CRP by using ROC curve for diagnosingMetS was0.215mg/dL,and its sensitivity, specificity were62.9%and51.3%,respectively. The optimal cutoff point of UA by using ROC curve fordiagnosing MetS was276.35umol/L,and its sensitivity, specificity were64.6%and52.8%,respectively.Conclusions In this study, positive correlation were observed between serum Hs-CRP,Hcy, UA and MetS,.but negative correlation were observed between serumNT-proBNP and MetS.Serum Hs-CRP and UA level were considered to have someextent value for diagnosing MetS.
