Objective:To study the methods of functional brain mapping;such asBold-fMRI,DTT,combination with neurophysiology technique:EEG andintraoperative electrical cortical stimulation (ECS) localization of languageareas with awaken surgery selectivity,assist to remove brain tumors infunctional areas of language.Methods: cortical mapping data from such as oxygen dependentfunctional magnetic resonance imaging (BOLD-fMRI) technology, usingthe block design and reciting task to activate broca area and paragraphcomprehension task to activate wernicke area, calculated cerebral lateralityindex (LI) to determine the dominant hemisphere;using of the electricalcortical stimulation (ECS) to localizate language areas under awakeanesthesia in patients with surgery. The data combined withneuronavigation assist microscopic remove braim tomors in the languageareas. Results: Functional cortical mapping was used for localization broca orwernicke area of16cases, image acquisition satisfied in16cases.Intraoperative awake and electrical stimulation of language areas in8cases,.Lesion total resection in10cases, subtotal resection in3cases, partialresection in3cases. Language function improved significantly after surgeryin6cases, unchanged in6patients, transient sensory aphasia in2case,deterioration of language function does in2case. Conclusion: Functionalbiain cortical mapping application in neurosurgery includings preoperativelocalization of language areas with BOLD-fMRI,DTT and intraoperativeelectrical cortical stimulation,has a good clinical value.Assisting with themincrease the extent of tumor resection and reduce postoperative morbidityand the lack of language features, to improve quality of life of the patients.
