Major depression is a common disease of Affective Disorder. Systematic studies on the treatment for major depression by electric acupuncture(EA) were performed in the aspect of experimental study and clinical study in this paper.Clinical studyThere are 30 patients accepted EA while 31 patients oral administrated maprotiline(MP). "Baihui" and "yintang" were used when accepting EA. "Yanglingquan" and "sanyinjiao" were selected for the patients of hepatic stagnation while splenic asthenia; "neiguan" and ''sanyinjiao'' were selected for cardiac-splenic asthenia; "taixi", "sanyinjiao" were selected for asthenic hepto-nephroyin. Patients in this group were given EA daily for 60 minutes for 6 weeks. Patients in the other group contain MP also for 6 weeks with daily dose from 75-250mg. The results show that:1. Both EA and maprotiline were effective in the treatment of major depression. For endogenous depression maprotiline is better. The scores of the syndrome of anxiety somatization and SAS of EA after treatment were significantly low^er than that of maprotiline. The score of Asberg Rating Scale of maprotiline group was significantly higher than EA group. Because maprotiline has more side effects than EA, so the efficacy index of EA group is significantly higher than maprotiline group.2. Blood plasma CORT and ET levels of depression were significantly higher than that of normal controls. Both EA and MP can reduce serum CORT and ET levels of patients and there exhibits no significant difference between this two groups before and after treatment.3. In depressive subjects, serum levels of IL-1 P , IL-6, TNF- a were significantly elevated when compared to normal controls. After antidepressant treatment of EA or Maprotiline , the levels of cytokines lowered accompany with the severity of depression. Not any significant difference was found between that two groups.4. There were abnormity of microcirculation of nail fold in depressive patients. The abnormity of blood vessel form of cardiac-splenic asthenia patients were so serious. The score of surrounding vessel of asthenic hepto-nephroyin patients were much higher than other types. EA improved bodily nail-bed microcirculation better than MP.Experimental studyThirty-six male Sprague-Dawley rats were randomly divided into there groups of 7 each: control group, model group and EA group . All rats except the control group singly housed and exposed chronically 21 days to a variety of mild unpredictable stressors, and the rats of EA group accepted electro acupuncture treatment at the same time. The resultsshow that:1. Compared with control group, the increased weight > the sucrose preference and activity of rats in open-field test in model group decreased significantly. EA can improve behavioral disorder of depressed rats2. The concentration of plasma ACTH and CORT, the number of AVP infected neurons in PVN increased significantly in depression-model rats compared to controls. Levels of ACTH, CORT and AVP markedly dropped in acupuncture group.3. Immuno-histochemistry results also showed that the number of infected BDNF neurons in hippocampus of depressed animals was much less than that of normal animals. EA can increase the number and improved the appearance of BDNF neurons in the depressed animals.4. Mild chronic stress increased the levels of NE and 5-HT in serum. It also decreased the levels of NE , 5-HT, DOPAC in prefrontal cortex and 5-HT, 5-HIAA in hippocampus. EA can decreased the levels of monoamine neurotransmitter in periphery and increase them in central nervous system.5. The Bmax and Kd were measured by radioligand binding assay. The result showed that chronic stress raised the Bmax and Kd of 5-HT2 receptor. The Bmax of 5-HT 1 receptor of model animals were lower than that of control. EA can up regulate the 5-HT1 receptor and down regulate 5-HT2 receptor.6. The levels of serum IL-1 P , IL-6 of model group were significantly higher than that of control.
