| Objective:To observe the effect of Chinese herbal medical-yisuling Jianji on insulin resistance (IR)of type 2 diabetes mellitus throng clinical and experimental research. Yisuling has the function to strengthen spleen .regulate liver, resolve phlegm and prompt the circulation of blood . The research demonstrated its functional mechanism at cellular and molecular level , and searched for the theoretical foundation for treating IR of type 2 DM. Methods:1. theoretical researchComprehensive roundup was made about IR of type 2 DM on the part of modern medicine and traditional Chinese medicine (TCM). On this basis .according to tutor' s experience and clinical practice, utilizing TCM theory, a conclusion was firstly drawn that depression of liver, deficiency spleen and the disorder of sputum and blood were the main pathological mechanism. As a result, the new method of strengthening the spleen, regulating the liver, resolving phlegm and prompting the circulation of blood was established. The theoretical research about Yisuling was completely done such as its theory, method, prescription, herbal constitution, etc.2. clinical researchA total of 120 patients with type 2 DM enrolled in the study. They were randomly divided into groups -the treatment group (Yisuling group ,60 cases) , the control group (metformin group ,60 cases), a treatment course last 2 months for each group . Before and after treatment, some indexes were observed including clinical symptoms, FBG, 2HPG in OGTT, INS, lipid, blood rheology , BMI, IR, IAI etc. Result : Yisuling could effectively reduce the patients' clinical symptoms, BG, lipid, regulate blood rheology, and increase insulin sensitivity. The total efficiency rate was up to 88. 3 percent,prior to control group 3. experimental researchYisuling could effectively reduce the BG of rats (deduced by STZ, plus hyperlipid diet ) with IR of type 2 DM, lipid, blood rheology and insulin. It improved insulin sensitivity, increased the capacity of N0> NOS, and enhanced expression of glucose uptake 4 mRNA in skeleton muscle. It also reduced the numbers of FAA, TNF-a and other IR factors, and improved the reaction on IR of model groups. Conclusion1. The result of clinical and experimental research showed that Yisuling had virtual effect on IR of type 2 DM .The mechanism was to improve the main segment of post-insulin receptor, regulate the metabolism of lipid and the index of blood rheology , and lessen the weight etc. It played general function at diverse targets and segments. 2. The research was found to be true and feasible , which provided scientificnew view and effective therapy. It founded good basis so as to offer thenew theory of treating IR of type 2 DM , improve clinical effect and developherbals with higher efficiency and lower toxicity.
