| PURPOSE: Diabetic nephropathy is the typical diabetes capillary complications and one of the main factors for death. The renal function of DN patients is developing to end stage renal failure, therefore the preventing and treating schemes research is significant. The study is portion of nation "shiwu" tackle key problem "study on prevention and treatment scheme of diabetic nephropathy renal failure",used randomly, single blind and control research method to scientifically observe and evaluated the Chinese herbs effects , and explore the relationships of syndrome and laboratory examination results. METHODS: All the cases were selected from Dongzhimen hospital of Beijing Chinese medicine university from Jan 2002 to Nov 2004, diagnosed as diabetic nephropathy correspond as period â…£ according to Mogensen method. The gender were not limited, the age from 27 to 79 years old. We used randomly, single blind and control research method, divided the patients to A group, B group and C group. The three groups were treated allopathy basically, A group treated with Chinese herbs of zhixiaotongmaining granule, zhixiaowenshenning granule and zhixiaobaoshenning granule on differentiate, at the same time Losartan balm; B group treated with Losartan, at the same time Chinese herbs balm; C group treated with Chinese herbs on differentiate and Losartan. We observed 3 months as one course and followed up observation. We observed syndromes, picture of tongue, pulse condition, 24hour urinary albumin, endogenous creatinine clearance rate, serum creatinine, blood urea nitrogen and others. We analysed data with SPSS10.0 statistical software. RESULTS: A group has 30cases, and 28cases were analysed. B group has 29cases, and 25cases were analysed, C group has 31cases and all were analysed. The effective evaluated in according with "the effective evaluated standard of chronic renal failure" in 'Clinical research principle of new herbal'. 1. Ailment effect evaluation All the treatment schemes are effective on diabetic nephropathy renal failure,Amongst the Chinese herbs scheme basically on nutrition therapy and subdue sugar therapy brought into 28cases,6patients (21.40%) showed excellent results,7 patients (25.00%) showed effective against,7 patients (25.00%) showed stability, 8 patients (28.60%) did not respond to the treatment, the total effective rate was 71.40%. The A scheme showed no marked difference with Losartan.. 2. Chinese medicine syndromes effect evaluation All the treatment schemes are effective on diabetic nephropathy renal failure syndromes, Amongst the Chinese herbs combined Losartan scheme basically on nutrition therapy and subdue sugar therapy brought into 31 cases, 3 patients (9.70%) showed excellent results, 26patients (83.90%) showed effective against,2patients (6.50%) did not respond to the treatment, the total effective rate was 93.5%. The C scheme showed advantages of Losartan. at syndrome evaluation,total integral evaluation and singleness evaluation. The A scheme was range between C and B scheme, and showed no marked difference with the two schemes. 3. Laboratory examination results evaluation The results showed Chinese herbs scheme basically on nutrition therapy and subdue sugar therapy improved creatinine clearance rate(p<0.05). All the schemes showed no more effect on serum creatinine, blood urea nitrogen,electrolyte and blood lipid(p<0.05),the reason was likely to the cases or the treatment courses were not enough. The three schemes reduced serum collagen â…£ remarkably(P<0.01,P<0.05). 4. Relationship analysis The results were not showing more relationship of the age, avoirdupois, blood press and Chinese medicine syndrome; the 24hour urine protein, serum creatinine,urea nitrogen and collagenâ…£ of yang-deficiency patients were higher than yin-deficiency and yin-yang deficiency patients, suggested that degree of illness of yang deficiency patients are more serious than yin deficiency and yin-yang deficiency patient.
