ObjectiveTo study and research the relevance of collagen type III in serum of obstructive nephropathy and renal function,so we could find a laboratory index to estimate the diagnosis value of PCIII and PIIINP concentration for the damage and recovery of renal function.Method①according to the result of ultrasound,thirty patients with half hydronephrosis that were final diagnosisd by IVP and ultrasound,were separated to three groups:Mild hydronephrosis group (10cases)、Moderate hydronephrosis group (10cases) and Severe hydronephrosis group (10cases),besides we choosed healthy as control group (10cases). PCIII and PIIINP concentration in serum were measured by radioimmunoassay (RIA); preoperative and postoperative Glomerular Filtration Rate were measured by nephro-dynamic imaging; to study and research the relevance between PCIII and PIIINP concentration in serum and GFR.②according to the result of GFR, the same thirty half hydronephrosis patients were also separated to four groups:A组(0-10mL/min)、B组(10-20mL/min)、C组(20-40mL/min)、D组(40-60mL/min).we did it in the same method.Results(一). according to the result of ultrasound: ①PCIII and PIIINP concentration in serum of three hydronephrosis groups entirely were all higher than the control group,and PCIII and PIIINP levels in serum were increasing gradually,with the degree of hydronephrosis classification is aggravating,the comparison between each hydronephrosis group and the control group,and the comparison between each hydronephrosis group had statistical significance (P<00.05);②Negative correlation was discovered between PCIII and PIIINP concentration in serum of three hydronephrosis groups with preoperative Glomerular Filtration Rate (r=-0.819, P<0.01);③the postoperative GFR was higher than the preoperative GFR of the Mild and Moderate hydronephrosis groups (P<0.05);and the postoperative GFR of the Severe hydronephrosis group did not recover obviously (P>0.05);(二). according to the result of GFR:①PCIII and PIIINP concentration in serum of four hydronephrosis groups entirely were all higher than the control group,and PCIII and PIIINP concentration in serum increased gradually with the decreasing of the glomerular filtration rate,the comparison between each hydronephrosis group and the control group,and the comparison between each hydronephrosis group had statistical significance (P<0.05);②Negative correlation was discovered between PCIII and PIIINP concentration in serum of three hydronephrosis groups with preoperative Glomerular Filtration Rate (r=-0.819, P<0.01);③the postoperative GFR was higher than the preoperative GFR of the group C and group D (P<0.05);and the postoperative GFR of the group A and group B did not recover obviously (P>0.05);ConclusionPCⅢ and PⅢNP levels in serum of hydronephrosis groups were increasing gradually with the degree of hydronephrosis classification is aggravating; Negative correlation was discovered between PCIII and PIIINP concentration in serum of three hydronephrosis groups with preoperative Glomerular Filtration Rate; So we presumed that the PCⅢ and PⅢNP in serum can be used to indicate the changes of the renal function of hydronephrosis,on condition that we depleted other organ fibrosis.
