Gap junction is the structure responsible for intercellular electrical impulse conduction. Recently, many studies have reported the phenomenon of " gap junction remodeling " in the setting of various heart disease such as myocardial infarction .ventricular hypertrophy and chronic heart failure. The term " gap junction remodeling " refers to the abnormal change of the size , number, density and spatial distribution of gap junctions under pathophysiologic conditions. The remodeling of gap junction may responsible for the increased incidence of ventricular arrythmia and sudden death in those heart disease because the change of gap junction can cause slowing of conduction velocity and nonuniform anisotropy which faciliate reentry. Gap junction remodeling is a complicated process which implicated abnomal change of multiple process including the regulation of connexin expression, assembly , degradation and so on. Modulation by phosphorylation of the connexin has been proved as an important factor for remodeling of gap junction in diseased heart. Under pathophysiology conditions, mechanical stimulation, endocrine factor and circulating hormones can activate specific kinase and /or phosphatases through complex signal passways , which may play an important role in the process of gap junction remodeling.In the above mentioned heart disease status (such as myocardial infarction , ventricular hypertrophy and chronic heart failure), elevated plasma B-type natriuretic peptide (BNP) has been reported. The elevated level of BNP is reported to be positively associated with the mortality and sudden death.
