Objective:To study the clinical changes of ANP,BNP,NT-proBNP levels and the changes of left atrial dimension and left ventricular ejection fraction by echocardiography before and after circumferential pulmonary vein isolation (CPVI) guided by the three-dimensional mapping system(CARTO) in patients with atrial fibrillation(AF), and the relationship ANP,BNP,NT-proBNP levels with left atrial dimension(LAD) and left ventricular ejection fraction(LVEF), also the predictors of recurrence after circumferential pulmonary vein ablation.Method:Twenty five patients with AF were enrolled in this study. Before the CPVI procedure and 3 months after successful CPVI. The plasma ANP, BNP, NT-proBNP levels were measured separately by radioimmunoassay and ELISA, LAD and LVEF were measured by echocardiography. The results were compar-ed with those of twenty five healthy people.Results:①Serum ANP level of before CPVI were higher than that in after CPVI and healthy people(both P<0.05); Serum BNP level of before CPVI were higher than that in after CPVI (P<0.01)and healthy people(P<0.05); Serum NT-proBNP level of before CPVI were higher than that in after CPVI and healthy people(both P<0.05); LAD of before and after CPVI were higher than that in healthy people(P<0.01); LVEF of healthy people were higher than that in before and after CPVI(P<0.01).②Before CPVI, the level of plasma NT-proBNP and LAD in patients with AF recurrence were higher than that in patients with non-AF recurrence; after CPVI, the level of plasma ANP and NT-proBNP in patients with AF recurrence were higher than that in patients with non-AF recurrence(P<0.01, P<0.05);③Before CPVI, there was a negative linear correlation between the levels of BNP and LVEF (r=-0.239), and there was a negative linear correlation between the levels of NT-proBNP and LVEF (r=-0.341);④In univariate analysis four variables were related to the recurrence of AF:age (OR:1.074,95%CI:0.964-1.197, P:0.195), BNP (OR: 0.995,95%CI:0.989~1.002, P:0.145), NT-proBNP(OR:0.977,95%CI: 0.956~1.997, P:0.028), left atrial dimension (OR:0.783,95%CI:0.629~0.973, P:0.027); In multivariate analysis NT-proBNP(OR:0.983,95%CI: 0.966~1.000, P:0.047) and left atrial dimension (OR:0.766,95%CI:0.587~0.999, P:0.049) were related to the recurrence of AF.Conclusion:The levels of ANP,BNP,NT-proBNP were significantly elevated in patients with AF, and decrease after CVPI. The elevated NT-proBNP level and left atrial dimension can predict the risk factor of AF after CVPI.
