ObjectiveTo investigate the differences of clinicopathologic characters,survival rates,mRNA and protein expression of PTEN and AKT between upper gastric carcinoma and lower gastric carcinoma, and discuss the significance of PTEN and AKT gene in the development of gastric carcinoma.MethodsThe clinicopathologic data and the outcome of the follow-up were analyzed retrospectively, including 73 upper gastric carcinoma (group U) and 366 lower gastric carcinoma (group L) patients. RT-PCR,SABC immunohistochemical stain and Western Blot were used to detect the mRNA and protein expression of PTEN,AKT and pAKT in tissue of 35 upper gastric carcinoma and 54 lower gastric carcinoma patients.ResultsThe clinicopathologic characters correlated with upper gastric carcinoma were male,diameter larger than 5 cm,diffused growth,Borrmann 3 type,early carcinoma,T3,T4,N2+N3 stage,undifferentiated type. Median survival time were 42 monthes and 60 monthes respectively in group U and group L,and 5-year survival rate in group U was significantly low than group L (P<0.01) .PTEN mRNA positive rate and brightness value was lower in gastric carcinoma than control (p<0.05) , and also lower in upper gastric carcinoma than lower gastric carcinoma (p<0.05) . AKT mRNA positive rate was higher in upper gastric carcinoma than lower gastric carcinoma (p<0.05) . PTEN protein expression and IOD value was lower in gastric carcinoma than control (p<0.05) , but pAKT protein expression and IOD value was higher in gastric carcinoma than control(p<0.05). PTEN protein expression and IOD value was lower in upper gastric carcinoma than lower gastric carcinoma (p<0.05) , but pAKT protein expression and IOD value was higher in upper gastric carcinoma than lower gastric carcinoma (p<0.05) .PTEN protein brightness value was lower in gastric carcinoma than control (p<0.05) , but pAKT protein brightness value was higher in gastric carcinoma than control (p<0.05). PTEN protein brightness value was lower in upper gastric carcinoma than lower gastric carcinoma (p<0.05) , but pAKT protein brightness value was higher in upper gastric carcinoma than lower gastric carcinoma (p<0.05) .ConclusionThe upper gastric carcinoma cases showed many differences comparing with lower gastric carcinoma, including more male, bigger tumer diameter, diffused growth, less early stage, later TNM stage, more lymphonodes metastasis, more undifferentiated type, less radical operation case, etc. The survival rate in group U was significantly lower than in group L.PTEN gene and protein expression were lower in gastric carcinoma especially in upper gastric carcinoma. AKT gene activity was higher in upper gastric carcinoma than lower gastric carcinoma. The pAKT protein expression was higher in gastric carcinoma, especially in upper gastric carcinom.
