The Aging Kidney Yang Deficiency Syndrome Diagnosis And Differential Expression Of The Gene Expression Profile Study

Objective:①To research the development and characteristic of theory of Kidney-Yang insufficiency syndrome②To establish the factors of syndrome differentiation and diagnosis criteria of Kidney-Yang insufficiency with old-aging③To search specific gene expression of Kidney-Yang insufficiency with old-agingMethods:①Consulting about more than 650 documents of TCM from Qin-Han Dynasty to recent time,and making a intensive research and summarization for the development and theoretical framework of syndrome theory of Kidney-Yang insufficiency. Exploring the essential content,theoretical framework and main characteristics of syndrome theory of Kidney-Yang insufficiency.②Creating Expert-Evaluation System of factors of syndrome differentiation by weight sorting of 23 experts which based on 40 factors of syndrome differentiation in early summary.③Designing questionnaire on epidemiology of syndrome,investigating syndrome of people with old-aging according to requirement of epidemiology,compared and anal sized epidemiology rule on syndrome on Kidney-Yang insufficiency with old-aging,compared with the result of Expert-Evaluation System.Obtaining factors of characteristic symptoms,common symptoms and complication symptoms, determine the distribution rule and diagnosis criteria on Kidney-Yang insufficiency with old-aging.④Based on the survey of epidemiology,we screened typical cases of Kidney-Yang insufficiency with old-aging,compared with the healthy people.Using gene chip technology,we researched the difference of gene expression profile of Kidney-Yang insufficiency with old-aging,and explored its clinical significance by using the methods of screening differentially expressed genes,classification of gene function,cluster analysis,pathway of molecular and so on.Conclusion:①summarized all the symptoms and syndromes of Kidney-Yang insufficiency from the past dynasties documents according to the 14 categories and 36 items.②Divided the diagnosis criteria of syndrome of Kidney-Yang insufficiency into three levels from the angle of scientific research,clinic and difficult and complicated cases.③For abtaining a more careful and accurate clinical differentiation,We classified syndrome of Kidney-Yang insufficiency into 12 subtype.④Formed factors of syndrome differentiation and diagnosis criteria of Kidney-Yang insufficiency with old-aging which contained three specific factors,eight common factors,ten reference factors that involved seven categories.⑤Took three cases of Kidney-Yang insufficiency with old-aging and three cases of healthy people,comparing their gene chip results by gene screening,cluster analysis, GO and Pathways gene analysis we got 20 up-regulated and 48 down-regulated differentially expressed genes,which mainly involved 13 signal pathway and function of neuroendocrine system,metabolism system and immune system.Discussion:①The development of theoretical system of Kidney-Yang insufficiency syndrome is based on abundant TCM theory and clinical practice.Its abundant connotation worth further research.②The combination of scientific research,clinic and difficult and complicated cases is essential to form a complete concept of diagnosis of Kidney-Yang insufficiency syndrome,which can enhance the level of syndrome differentiation.③The Kidney-Yang insufficiency syndrome is still basal,compound and dim,and it need to be refined to guide the clinical works.④The forming of rough diagnosis criteria of Kidney-Yang insufficiency syndrome is based on the questionnaire on epidemiology of syndrome and the analysis of expert system.This kind of preliminary attempt of research method is need to be deepened.⑤The abnormality of the control network of neuroendocrine system,metabolism system and immune system may explain the key mechanism of the development of Kidney-Yang insufficiency with old-aging.The disorder of neuroendocrine system and multiple metabolism system are the main links of the development of Kidney-Yang insufficiency with old-aging.
