Speech rehabilitation rises gradually in our country in recent years. As one important branch of speech disorders, dysphonia is getting more and more attention. A comprehensive and objective assessment is the key of the treatment of dysphonia. With the development of computer technology, the assessment tools of phonation are increasingly rich, which gives strong support for making phonation assessment more and more comprehensive and objective. On the basis of previous research of phonation at home and abroad, this thesis summarizes the problems in current speech assessments in clinical application. This thesis puts emphasis on discussion about breath and vioce assessments and clarifies the necessity of general phonation. Two kinds of research have been done separately focus on continuous phonation and intermittent phonation and then to verify the research results in clinical application and to observe their validity. Some proposals are given to resolve the problems in current speech assessments. This study obtains some valid research results and gives more parameters for improvement of phonation assessment, discussing the rule of objective measurement and pushing the development of speech rehabilitation.ObjectivesAccording to the existing evaluation to analysis the proportion of special children respiratory and voice disorders; to analyze the influencing factors of the testing requirements and to get the standard of the requirement of testing and value standard on general phonation conditions evaluation; To select sensitive parameters of general phonation through several kinds of research and experiences and to discuss their meanings in clinical application; To discuss the characteristics of preschool children's general phonation; according to the phonation assessment results of children with hearing impairment, to discuss the validity of the sensitive parameters and to instruct dysphonia rehabilitation in clinical application. Methods1. According to the issues raised by the review of the study, to analyse assessment results of218clinical children with speech disorders and to summarize the clinical characteristics of children's speech disorders and focus on dysphonia incidence and discussion about difficulties in assessment.2. On the basis of previous studies, to divide general phonation into continuous phonation and intermittent phonation to facilitate the study carried out. Influence factors are studied by selecting adults with good controllability as research subjects.3. To analyze acoustic characteristics of language material of continuous phonation and intermittent phonation by using "phonation-measuring instrument (PHONATION)"; to find sensitive acoustic parameters by comparing and analyzing several different parameters.4. To explore characteristics of preschool children's general phonation; to discuss the validity of the sensitive parameters and the clinical meaning compared to characteristics of hearing-impaired children's general phonation.ResultsPre-school children are the major in speech impairment groups; the incidence of breath and dysphonia is up to75%of speech disorders; Respiration method, phonation method and speech rate can significantly affect some parameters of general phonation; The changes of the support of respiration and the muscle tension of phonation can significantly affect the maximum phonation time(MPT), fundamental frequency standard deviation(FO), and jitter; The changes of the support of respiration and speech rate can significantly affect the average of syllable time, the standard deviation of syllable, the average of interval time, the standard deviation of syllable and the efficiency of phonation.ConclusionsPre-school period is the critical period for children's phonation rehabilitation; It is necessary to use normal assessment requirement when assessing continuous phonation and intermittent phonation in order to ensure the parameter results effectiveness; Selecting multiple parameters can fully reflect the characteristics of the two kinds of phonation; Assessing the parameters of the two phonation in preschool children have different characteristics; when use them, as appropriate, to consider changes in the physical development; assessment parameters can basically reflect the difference between phonation of hearing-impaired children and normal children, can provide effective guidance for assessment and treatment of dysphonia.
