The pathogenesis of osteoarthritis is still unclear, but what we can sure is that the pathological is degeneration of cartilage. China owns the largest population in the world, and also the number of elderly population is more larger than any other country in the world, but the data of epidemiological investigation about Hunan province is very little. In order to reduce and avoid the risk factors associated with osteoarthritis and reduce pain and improve function in patients, alleviate the economic burden of patient, improving the quality of life patients with osteoarthritis is of great significance. It became necessary and urgent to conduct further research on osteoarthritis.Objective Based on prevalence survey of osteoarthritis in middle-aged and elderly people in urban and rural areas in Hunan province, find out middle-aged and elderly osteoarthritis prevalence characteristic, risk factors, Symptom severity and provide all kinds of possible risk factors and discuss the risk factors of knee osteoarthritis. In order to further improve Hunan area middle-aged and elderly people osteoarthritis epidemiological data provide reference.Method Hunan local registered permanent residence,live for a long time (≥ten years),aged≥40years old and disease, whether or not limited to. Used to design a questionnaire, including basic knowledge and risk of osteoarthritis of the knee, on controlling the disease in middle-aged and elderly population correlation between knowledge and the will, knowledge of approaches, comparative understanding of related diseases in four areas.Using stratified-cluster-multistage sampling method, Hunan area was divided into two layers which are urban and rural areas, and then randomly selected two community committees in a district (town). The research was door-to-door surveys to investigate demographic data of the local residents and the condition of the primary knee osteoarthritis. Excel and data entry, establishing a database with Epidata3.0, SPSS17.0statistical software for data analysis, analysis data was used the chi-square test and logistic regression analysis.Result A total of2471issued questionnaires in Hunan area, recycled paper2300, effective recovery93.1%papers. In this survey, The total prevalence of primary knee osteoarthritis in elderly people in central region of Hunan province is20.3%. Standardized prevalence rate of16.6%urban, rural23.4%, rural OA prevalence was significantly higher than urban, Comparison of urban and rural areas of different ages,40,50old age group the prevalence in rural than urban (P<0.05),60years old age group the prevalence of OA was no significant difference between urban and rural areas.Overall Prevalence was higher tendency to increase with age. Aging, obesity and occupation were risk factors for OA. Body mass index was positively correlated with the incidence of OA. Smoking, high labour intensity, work attitude and OA family history were also risk factors.the severity of old patients was more serious than younger, and the statistics had significant statistically(P<0.05); The severity of women patients was more serious than men, and the statistics had significant statistically(P<0.05); the severity of rural patients was more serious than urban, and the statistics had significant statistically(P<0.05); the severity of higher body mass index patients was more serious, and the statistics had significant statistically(P<0.01); the severity of heavier works patients was more serious, and the statistics had significant statistically(P<0.05); the severity of smoker patients was more serious than non-smoker, and the statistics had significant statistically(P<0.01). Sex, body mass index and labor intensity were the risk factors of primary knee osteoarthritis in elderly people in Hunan area.Conclusion1. The prevalence of primary knee osteoarthritis in elderly people of urban and rural areas in central region of Hunan province is 20.3%. The prevalence in urban areas is16.6%and23.4%in rural areas.2. Hunan area, OA is very common in women, obese people, rural population and high labor intensity crowd.3. Hunan area,the severity of female, old people, rural areas, obesity, high labor intensity and smoking patients with knee OA is worse.4. Hunan area, grass-roots community middle-aged and a general lack of awareness associated with osteoarthritis of the knee, especially lack of knowledge of prevention. |
