| BackgroundsOsteoarthritis is a common disease in the elderly group, which seriously affected the quality of life of the elderly people and cost huge medical expenses. With the aging population gradually increasing, this issue has become more serious. Osteoarthritis is one of the main reasons resulted in the people over the age of 50 years for the loss of their labor ability, and it also brought huge the social and economic burden. World Health Organization (WHO) started a global scale event on January 23,2000, which was intended to cause governments, medical research institutions, the public and the community focus on musculoskeletal disorders--the "Bone and Joint Decade" campaign, in which including osteoarthritis. The pathogenesis of osteoarthritis is still unclear, but what we can sure is that the pathological is degeneration of cartilage. China is the world's most populous country, and also the number of elderly population is much more than any other country in the world, but the data about our country is very little. In order to improve the quality of life of the elderly population group, save the Government's medical expenses, it is necessary to conduct further research on osteoarthritis.To understand epidemiological characteristics of the symptomatic osteoarthritis, we conducted a sampling survey, by taking in the local people aged over 40, at the same time, we took X-ray film projection with the people who had osteoarthritis symptoms. The result would be a good representation for the future Osteoarthritis research in Guangzhou, and it could provide the related data for us to prevent and treat Osteoarthritis.ObjectivesTo investigate the distribution of prevalence of symptomatic osteoarthritis(SOA), assosiated factors and other items in middle-aged and senior residents in district of Guangzhou, and provide the basic data for the prevention and treatment of osteoarthritis.Methods1. From November 2007 to January 2008, using stratified-multi-steps-cluster sampling method, the basic sampling unit is the neighborhood committees and village committees, we performed the survey in Guangzhou including urban and rural areas with the people who have a formal account with the age above 40 years, we also conducted a questionnaire survey to investigate the subject. The subject were informed of the consent before survey. All the investigations were conducted by the Epidemiology and Orthopedic specialists. The questionnaire was conducted according to the unified "Bone and Joint Health Survey questionnaire".The subject were undergone the X-ray fluoroscopy who have the symptoms of osteoarthritis with the corresponding parts of body. Radiological check were completed by the dedicated radiology physicians, X-ray film diagnose were performed by orthopedic doctors. The diagnostic process had always followed the randomized, controlled, double-blind principle. Main investigation index measures:The prevalence rate, gender, age of symptom osteoarthritis of aging people of Guangzhou region, and the distribution of osteoarthritis in various parts of body. Statistical analysis:Analysis was performed using EpiData 3.1 and SPSS 13.0 software for data processing and analysis, measurement data indicated by (?)±s and the enumeration data was using x2 test, P<0.05 indicates significant meaning.Results1. A total of 1342 cases were included in the survey, male 639 cases and female 703 cases, the gender ratio is 0.91:1.00, Men and women aged were (58.0±11.4) years and (56.8±10.9) years respectively, the height of the men and women were (165.6±6.0) cm, and (155.4±5.9) cm respectively, the body weight of the men and women were (60.6±8.6) kg and (54.0±8.9) kg respectively, and the BMI index of the men and women were (22.1±2.7) kg/m2, and (22.1±3.3) kg/m2 respectively.2. The total incidence of symptomatic cervical spine osteoarthritis was 23.2%, and male incidence and female incidence were 15.5% and 30.2% respectively, The prevalence of female was significantly higher than men (x2=40.426, P<0.01); And the total incidence of symptomatic lumbar spine osteoarthritis was 27.0%, and male incidence and female incidence were 19.9% and 33.6% respectively, and also the prevalence of female was significantly higher than men (x2=31.819, P<0.01). The total incidence of symptomatic hand osteoarthritis was 11.4%, and male incidence and female incidence were 8.6% and 13.9% respectively, The prevalence of female was significantly higher than men (x2=9.245, P<0.01); The total incidence of symptomatic knee osteoarthritis was 19.0%, and male incidence and female incidence were 12.7% and 14.8% respectively, The prevalence of female was significantly higher than men (x2=31.711, P<0.01)3. The SOA prevalence of cervical spine, lumbar spine, hand and knee are all increased with the age, and all have the high prevalence of the symptomatic osteoarthritis at 70-79 age group.4. Cervical 5/6 segment had the highest prevalence of SOA for 21.0%, followed by the neck 6/7 segment, and 4/5 segments,17.8% and 17.5% respectively. Lumbar 4/5 segment had the highest prevalence of SOA for 23.3%, followed by the lumbar 3/4 segment, and L5/S1 segments,21.6 % and 20.1 % respectively. DIP had the highest prevalence of SOA for 9.8%, followed by the PIP and MCP segment,7.2% and 3.1% respectively. PFJ had the highest prevalence of SOA for 15.0%, followed by the medial TFJ and lateral TFJ,14.5% and 13.0% respectively.5. There was a significant positive correlation between the age and imaging grade of cervical spine, lumbar spine, hand and knee, with the greater age, the imaging performance of the symptomatic osteoarthritis became more seriously.Conclusions1. The total incidence of symptomatic cervical, lumbar spine, hand and knee osteoarthritis was 23.2%,27.0%,11.4% and 19.0% respectively.2. The SOA of cervical spine of the elder people over 40 yeara old in Guangzhou mostly occur in the cervical of 5/6 segment, and lumbar osteoarthritis mostly occur in the lumbar of 4/5 segment. The hand SOA mostly occur in the DIP segment. The knee SOA mostly occur in the PFJ segment.3. There was a significant positive correlation between the age and imaging grade of cervical spine, lumbar spine, hand and knee, with the greater age, the imaging performance of the symptomatic osteoarthritis became more seriously. |
