Theoretical & Clinical Research In Treating Hypothyroidism Based On Academic Ideas On Asthenic Disease By Synopsis Of Golden Chamber

Objective:To study Zhongjing’s academic thoughts in the differentiation and treatment of asthenic diseases and search the law of theory in treating hypothyroidism,based on the ideas of Synopsis of Golden Chamber,through the clinical research in treating hypothyroidism with modified Shuyu pills, Shuyu pills is a very famous prescription on treating asthenic diseases, to observe the results of both the clinical symptoms and the biochemical indicators.Methods:�'�By studying TCM literature related to consumption diseases,generalize representative views by some major medical experts.�'� Analyse in detail the original text of Synopsis of Golden Chamber,collect the specific provisions involving consumptive diseases and classify them according to etiology and pathogenesis, therapeutic principle and therapies and herbs. Based on all of these, sum up the law of theory in treating consumptive diseases in terms of Synopsis of Golden Chamber.�'� Explore the law of theory in treating hypothyroidism,based on asthenic disease treated by Synopsis of Golden Chamber.�'� Clinical research:By random testing means, 120 cases were randomly divided into 3 groups,40 cases in the group of traditional Chinese medicine, 40 cases with western medicine treatment,and 40 cases combined traditional Chinese and Western medicine. Western medicine group took left-handed thyroxine tablets, Chinese traditional medicine group was provideded with Shuyu pills changed into medical broth, and the combined group used both left-handed thyroxine tablets and Shuyu pills. Before and after the treatment, the patients were observed carefully about their clinical symptoms, signs, thyroid function and blood fat index to evaluate the comprehensive curative effects. At the end of the eighy weeks course of treatment, accorting to standard out-fall-off, efftectively selected cases were counted.Excluding the 9 cases of no time or follow-up data insufficiency, at last, 112 valid cases were collected meeting the requiremnts of the study cases.Among them the the shuyu pill group is 38 cases, the western medicine group is 37 cases,the joint group is 37 cases. Input those related date into computer for statistical processing, analysis and evaluation of the cllical efficacy of the three groups before and after treatment.Results:�'�By indepth study of Synopsis of Golden Chamber, we found that Zhongjing’s thoughts inherited the statements on the consumption disease in Huangdi Neijing and Nanjing, and further developed the consumption theory. There are detailed dissertation on the principle, method, formula and herbs of the consumption disease. The main causes of consumption disease include: being weak at birth, improper diet,working too hard, suffering from serious diseases or lasting too long and lack of good rest when recovering. Especially being weak at birth, too much sexual life and over-thinking are the most common causes. The formation of the consumption disease takes a long time and the mechanism is quite complex,characterized by weak blood and Qi or blood and Qi stock. The symptom shows that the pulse feels strong or extremely weak and that Yin and Yang are either too weak or strong.The treatment principle is to strengthen the weak, especially nourish spleen and kidney, and warm and build up Yang Qi.The basic pathogenesis of hypothyroidism is deficiency in spleen and kidney, cold within the body, mainly reflected in Yin, Yang, Qi and blood deficiency. The treatment rules of hypothyroidism are as follows:nourish spleen, stomach and build up inner Qi, warm kidney Yang and supplement natural deficiency,emphasize on sweet and warmth and strengthen the body resistance to eliminate evils,coordinate both deficiency and excess, and build up both Qi and blood.�'�General situation of the three groups such as sex, age, the course of disease and the disease severity before treatment were no significant differences.�'�The comparion of disease effect in three groups: the totl effective rate of the joint group was 91.89%,of the modified Shuyu pills group was 65.79%,of the westen medine group was 78.38%, the totl effective rate of the joint group was higher than the modified Shuyu pills group and the westen medine group, P<0.05, the totl effective rate of the modified Shuyu pills group and the westen medine grougp was different significantly, P<0.05.4. The comparion of syndrome effect in three groups: the totl effective rate of the joint group was 94.59%,of the modified Shuyu pills group was 76.31%, of the westen medine group was 59.45%, the totl effective rate of the joint group and the modified Shuyu pills group were better than the westen medine group, P<0.05, the totle effective rate of the joint group and the Shuyu pills group was different significantly, P<0.05.5.The comparison of FT3、FT4 、TSH: The result showed that the westen medine group was better than the the modified Shuyu pills group, P<0.05, the joint group was better than the westen medine group and the modified Shuyu pills group, P<0.05.6. The comparison of TPOAB、TGAB:The result showed the joint group and the modified Shuyu pills group were better than the westen medine group, P<0.05.7. The comparison of TC、TG、LDL、RBC、HGB:The result showed that the joint group was better than the modified Shuyu pills group and the westen medine group, P<0.05; as for TC、TG、LDL, the joint group and the modified Shuyu pills group were better than the westen medine group, P<0.05; as for HDL、RBC、HGB, the joint group and the modified Shuyu pills group were better than the westen medine group, P<0.05.Conclusion:1.The treatment rules of hypothyroidism: nourishing spleen, stomach and middle qi, focusing on nourishing kidney yang, emphasizing on sweet and warm drug nourishing yang, strengthening body resistance to eliminate evil, treating both deciency and excess, focusing on nourishing spleen and kidney, treating both the qi and blood.2. Clinical observation showed that the modified Shuyu pill can not only effectively improve the signs and symptoms of patients, and has effects of enhance the serum FT3, FT4, reduce the TSH, reduce the TPOAb and Tg Ab, reduce blood fat, improve the effect of anemia, etc.
