BackgroundPrimary trigeminal neuralgia,also known as "pain cramps",is internationally recognized as one of the complicated diseases,trigger by either lateral facial trigeminal nerve distribution of recurrent episodes of severe pain as the dominating pain.The current drug treatment is mainly repressed the symptomatic pain,which can leads patients with long-term medication,encountered much side effects,and failed to eliminate the pain forever.Catgut embedding therapy at acupoint(??????)has advantages which prolong stimulation,without repeated operation of buried in the cerebral cortex after new establishment of excitation foci,further inhibit the original pain to excite,and thus play a role as pain eliminator.In order to further explore better treatment of primary trigeminal neuralgia,so this study is observed electro-acupuncture combined with acupoint catgut embedding,and applied Dong's Odd acupoints(????)for the prime trigeminal neuralgia which already demonstrated definite therapeutic effect at the lateral three points,inferior-side three points,which the catgut embedded.Purpose:The aim of this study was randomization observation the clinical efficacy of electro-acupuncture together with catgut embedding therapy at acupoint for facial pain treatment(primary trigeminal neuralgia)which enabled to explore its scientific,feasibility and pathogenesis.METHODSAccording to the order of treatment were randomly divided into this study,with strictly comply with the scientific research methods needed;clinical observation from July 2015 to March 2017,at the No.2 ward of acupuncture and rehabilitation,in the First Affiliated Hospital of Guangzhou University of Traditional Chinese Medicine.Patients were randomly divided into three groups:Group A(acupoint catgut embedding group),group B(conventional electro-acupuncture group)and group C(western medicine group).Evaluation Criteria:Follow the methodological techiques from "clinical disease diagnosis and efficacy criteria" wrote by Weiyan Wang,to determine the efficacy criteria for patients before and after treatment evaluation,Improvement procedure set up according to "Neurology-3rd Edition" wrote by Jiang Wu.The pain suppression marks for primary trigeminal neuralgia and data from visual analogue scale(VAS),were the basis for the TCM evaluation,the short-term efficacy evaluation(the day after the end of the treatments),the long-term efficacy evaluation(one month after the treatments)to evaluate and analyze the clinical efficacy of electro-acupuncture combined with acupoint catgut embedding therapy for facial pain(primary trigeminal neuralgia).Acupuncture treatment:Group A(acupoint catgut embedding group)Catgut embedding therapy at acupoint.Group B(simple electro-acupuncture group)Electro-acupuncture treatment:the master points(??)and adjunct acupoints(??)treatment,power mode:with dense wave pattern,the current intensity keep to the patient tolerate limits,only applied electro-acupuncture treatment,not any catgut embedding.Group C(western medicine group)oral intake of carbamazepine.Treatment Course:group A(acupoint catgut embedding group):Acupoint catgut embedding therapy,the two point catgut embedding for two weeks,a total of three treatment courses,the time was for six weeks,the day after the three treatment and one month after the evaluation of curative effect.Group B(conventional electro-acupuncture group):The treatment was 2 times per week,the next day or every other day for a total of 1 times,a course of treatment for a total of three courses,the time was for six weeks,the day after the twelfth treatment and one month after the evaluation Group C(western medicine group):oral intake carbamazepine daily,continuous uninterrupted medication for six weeks.Evaluation of efficacy of the last day of the sixth week and a month after the completion of the treatment courseResultThe analysis results show that More recently,the overall effect of 1 patients in three groups:group A was 93.33%,B group was 86.67%,the effective rate of group C was 80%.The overall efficacy of the three groups of patients was checked by the Fisher exact probability method,and the total effective rate of the three groups was statistically different between the two groups in P<0.05.But the multiple comparisons between groups need to correct the test level,and the correction method is as follows:Alpha = alpha/(K(k-1)/2 + 1)Among them,K is the number of groups,so alpha '=0.0125'.According to the"=0.0125 alpha test level,can have effect between group A and group C were statistically significant,multiple comparisons among the other groups were not statistically significant.2,three groups of patients before and after the recent symptoms of Chinese medicine treatment before and after treatment points:The single factor analysis of variance,P>0.05 score of the three groups of TCM symptom score before treatment,no significant difference between the three groups;TCM symptom scores after treatment and the difference between before and after treatment by single factor variance analysis,P<0.05,there were significant differences,further TukeyHSD test were carried out between multiple comparisons;In the three groups of TCM symptom scoring group,the scores before and after treatment were compared by paired t test,and there were statistically significant differences between the three groups,that is,the treatment methods were effective in both groups P<0.05;The comparison between group A and group B between the groups before and after treatment,after treatment and the difference was statistically different,the effect of the two groups are different.3,three groups of patients with short-term visual analog score comparison:The three groups before treatment in visual analog scale by single factor variance analysis,P>0.05,no significant difference between the three groups after treatment;the recent visual analog scores before and after treatment and the difference between the single factor analysis of variance,P<0.05,statistically,further TukeyHSD test for multiple comparisons between groups;In the three groups,the short-term visual analogue scores were compared by paired t test before and after the treatment.There were statistically significant differences between the three groups,that is,the treatment methods were effective in both groups P<0.05;The TukeyHSD between the three groups after the treatment of multiple comparison showed that the recent visual analog scores before and after treatment and the difference compared with the other two groups were statistically significant.4,three groups of patients:group a long-term overall efficacy rate was 90%,B group was 75.17%,the effective rate of group C was 80.22%.The long-term overall efficacy of the three groups was checked by the Fisher exact probability method,and the P>0.05 and the three groups had no significant difference in the effective rate.5,three groups of patients with long-term Chinese medicine symptoms before treatment and after treatment points:The single factor analysis of variance,P>0.05 score of the three groups of TCM symptom score before treatment,no significant difference between the three groups;TCM symptom scores after treatment and the difference between before and after treatment by single factor variance analysis,P<0.05,there were significant differences,further TukeyHSD test were carried out between multiple comparisons;In the three groups of TCM symptom scoring group,the scores before and after treatment were compared by paired t test,and there were statistically significant differences between the three groups,that is,the treatment methods were effective in both groups P<0.05;The comparison between groups,group A and group B,group C(P<0.05)was statistically significant difference between group A and group C,and before and after treatment the difference was also statistically significant(P<0.05),the effect is better than that of group B and group C respectively.The long term visual analogue score was compared between the 6 and three groups:The three groups before the treatment of long-term visual analogue scores before and after treatment and the difference is the single factor variance analysis and comparison,P>0.05,no significant difference;simulation score by single factor analysis of variance,P<0.05 forward vision of three groups after treatment,there were significant differences,further TukeyHSD test were carried out between multiple comparisons;In the three groups,the long-term visual analogue score was matched by paired t test before and after treatment,and there were statistical differences between the three groups,that is,the treatment methods were effective in both groups P<0.05;The three groups of TukeyHSD multiple comparison showed that the treatment of long-term visual analogue score than other two groups were statistically significant.Conclusion1,group A(electro-acupuncture combined with acupoint catgut embedding),group B(simple electro-acupuncture group),group C(oral intake western medicine group),three groups of treatment methods showed effectiveness.2,Group B and group C were effective,while group A had better result than group B and group C,especially group C.3.electro-acupuncture combined with acupoint catgut embedding group,applied catgut embedding with Dong's Odd acupoints(????)at lateral Sanli point,inferior-side Sanli point,can improved the efficiency,the value of clinical application.4.Electro-acupuncture combined with acupoint catgut embedding treatment of facial pain(primary trigeminal neuralgia),can effectively alleviate the pain,after all,it is a simple,safe and effective method.
