Research On The Influencing Factors Index System Of Doctor-Patient Relationship Of Traditional Chinese Medicine Medical Institutions

In the interactive activities of human society,any relationship is a social construction under the restriction of rules,and the relationship between doctors and patients is no exception.The harmony between doctors and patients is one of the important factors that directly affect the level of medical development and the level of treatment.Currently,the literatures on doctor-patient relationship focus mainly on the attribute and mode of doctor-patient relationship,the influencing factors of doctor-patient relationship,and how to build a harmonious doctor-patient relationship and other perspectives.The researches on the influencing factors index system of doctor-patient relationship mainly use the qualitative method or self-made questionnaires,and no combination of qualitative and quantitative methods is used in these researches,and the index system rarely reflects the influence of TCM culture on doctor-patient relationship.Therefore,it is of great importance to construct the influencing factors index system of doctor-patient relationship of TCM medical institutions that have the characteristics of "the sincerity of great doctors" and"holistic medical mode of TCM" as the core of TCM culture speciality.Firstly,the literature research method,qualitative interviews,comparative research method and Delphi expert consultation method are the four main methods used in this research to analyze and summarize the influencing factors of the doctor-patient relationship and to establish a basis of formulating the questionnaires on doctor-patient relationship for doctors and for patients.Secondly,the method of literature research and qualitative interviews are also used to research on the correlation between TCM culture and doctor-patient relationship from five aspects including traditional Chinese medicine and ancient philosophy,the connotation of Confucian thought,the traditional medical ethics and medical mode,the ancient traditional doctor-patient relationship,and the current humanitarian significance of traditional medical ethics.By researching on these,the influencing factors of TCM culture on doctor-patient relationship are analyzed and thus the guidance for formulating the questionnaires on doctor-patient relationship for doctors and for patients is provided.Thirdly,questionnaires are developed to study the reliability and validity.The questionnaires conclude six dimensions including medical system,medical quality,service level,medical expenses,comprehensive situations of the patients,and TCM and traditional culture.The items related with the solutions for improvement of doctor-patient relationship are also included and doctors as well as patients are surveyed simultaneously.The statistics of 1459 questionnaires from patients and 689 questionnaires from doctors are collected and analyzed,and comparative research method is used to analyze the perceptions of doctors and patients on the influencing factors and solutions for improvement of doctor-patient relationship.This provides evidence for the establishment the influencing factors index system of doctor-patient relationship.Finally,on the basis of the above research,the analytic hierarchy process(AHP)is applied to establish the hierarchical model,and the influencing factors index system of doctor-patient relationship of TCM medical institutions is constructed.The main conclusions are as follows.1.The traditional Chinese medicine culture,namely the thinking mode of TCM and "the sincerity of great doctors",has a profound impact on doctor-patient relationship.China's traditional medical ethics of "the sincerity of great doctors" has a real humanitarian significance in the current era and promoting the traditional medical ethics has a profound significance in constructing a harmonious doctor-patient relationship.2.The data from the survey of medical institutions show that the professionalism and objectivity of the media,the accuracy of disease diagnoses,whether doctors fully explain the condition and treatment measures,the ways and the proportion of the payment of medical expenses,whether doctors and patients have effective communication as well as the traditional medical ethics and culture are the most important influencing factors of doctor-patient relationship.For the improvement of doctor-patient relationship,the medical staff interviewed believe that increasing government investment is most important.3.The data from the survey of patients on doctor-patient relationship show that the construction of doctors' professional ethics,the accuracy of the diagnoses given by hospitals,whether doctors fully explain the condition and treatment measures,the ways and the proportion of the payment of medical expenses,whether doctors and patients have effective communication as well as the traditional medical ethics and culture are the most important influencing factors of doctor-patient relationship.For the improvement of doctor-patient relationship,the patients interviewed believe that the support from the policies of healthcare reform is most important.4.The analytic hierarchy process(ABP)is used to establish a hierarchical model,which is divided into the layer of decisive targets,the layer of intermediate factors and the layer of alternative plans.According to the research results of surveys of patients and medical institutions on the influencing factors of doctor-patient relationship,the order of the weight of every factor's influence on the decisive targets is calculated.Based on the order of the weight,the comprehensive index system of the influencing factors of doctor-patient relationship of TCM medical institutions is established.The doctor-patient relationship has been a concern of government and society and is related to public health and thus a major issue of livelihood.In this research,a combination of qualitative and quantitative methods is used to systematically analyze and summarize the influencing factors of doctor-patient relationship and the influence of TCM culture on doctor-patient relationship.The analytic hierarchy process(AHP)is used to establish a hierarchical model,and the perceptions of doctors and patients on the influencing factors of doctor-patient relayionship are researched respectively in order to define the weight of the influencing factors and to construct the comprehensive index system of the influencing factors of doctor-patient relationship of TCM.This research provides evidence as support for promoting the harmonious doctor-patient relationship and forming policies,and is thus of great importance in assuring the public's rights of health.
