Study On The Mechanism Of Renal Interstitial Fibrosis Treated By MicroRNA And The Effect Of Heshenluo Granule On Chronic Renal Failure

Objective 1.Clinical observition To observe the clinical effect of heshenluo capsule.2.Experimental study To explore the possible mechanism of heshenluo capsule in the treatment of chronic renal failure based on microRNA level.Methods 1.Clinical observition Select 80 cases of patients with chronic renal failure,divided into treatment group and control group randomly.The two groups of patients were given granules of heshenluo and niaoduqing Correspondly for 3 courses of treatment.Observe the changes of clinical symptom,the routines of blood,urine,biochemical and the microRNA of urinary sediment during the treatment.2.Experimental study(1)The model of unilateral ureteral obstruction in rats were regarded as the object of study.Pathological renal tissues were taken from the rats taking the granules of heshenluo for 4 weeks.Differential expression of microRNA were screened out by microRNA chip technology.(2)HK-2 cells were the object of the study,adding serum containing granules of heshenluo during the process cell culture.Detect the expression of ?-SMA?E-cadherin?ZEB1?ZEB2?mRNA and microRNA-141.(3)Transient transfection of HK-2 cells by mimic and inhibitor of microRNA-141,at the same time the serum containing granules of heshenluo were added into the culture medium of HK-2 cells.Observe the above laboratory indicators too.Results 1.Clinical observition ? The two drugs can reduce the level of Scr?BUN?Ccr in patients,but the granules of heshenluo is better than that of niaoduqing(P<0.05).? The two drugs can reduce the level of TC?TG,but the granules of heshenluo is better than that of niaoduqing(P<0.05).? The two drugs can rise the HGB level of patients,but the granules of heshenluo is better than that of niaoduqing(P<0.05).? The two drugs can rise the miRNA-141 level of patients,but the granules of heshenluo is better than that of niaoduqing(P<0.05).? The two drugs can improve clinical symptoms of patients,and the granules of heshenluo is better than that of niaoduqing(P<0.05).The total effective rate in treatment group is 80%is better than that in control group 59%(P<0.05).2.Experimental study(1)The number of differential expression of microRNA is 26,containing up-regulation 16,down-regulation 10.(2)? after 36h,the expression of ?-SMA?mRNA are significantly increasing,and the expression of E-cadherin?mRNA are significantly decreasing,conforming to renal interstitial fibrosis.? serum containing granules of heshenluo can decrease the expression of ?-SMA?ZEB1?ZEB2 and mRNA,increase the expression of E-cadherin?mRNA and microRNA-141.(3)serum containing granules of heshenluo can decrease the expression of ?-SMA?ZEB1?ZEB2 and mRNA,increase the expression of E-cadherin?mRNA and microRNA-141,can effectively reverse the changes of HK-2 cells.Conclusion 1.Clinical observition The granules of heshenluo can effectively improve the patient's symptoms and laboratory indicators,exist effectiveness in the treatment of chronic renal failure.2.Experimental research The treatment mechanism of heshenluo granules is complexity,which be controlled by multiple targets.The granules of heshenluo can effectively reverse the transdifferentiation of renal tubular epithelial cells,which may be achieved by activating the loop of microRNA-141/ZEB1/ZEB2.
