Gas explosion accident,as one of the top five coal mine disasters,seriously affects the status of coal mine safety production in China.It is necessary to make a comprehensive causation analysis and provide a reference for the prevention of such accidents.Through the review,it is found that the research method is not systematic in the causation analysis of the gas explosion accident.In most researches,the methods of experience induction,single accident case summary,observation,literature combing,interview are adopted,which show a strong subjectivity or overgeneralization.In the next place,a large number of causation analysis of unsafe acts and unsafe physical condition have been taken,however the causation analysis of other aspects is less.Therefore,a systematic gas explosion accident cause analysis method is established to exhaustively analyze the causes of human factors,physical factors,organizational factors and the external factors,which provide a more comprehensive reference for the risk assessment and accident prevention of the gas explosion accident.The thesis uses the methods of literature review and the contrastive analysis to determine the theory of the accident case study and the coal mine gas explosion accident causation classification system was established based on the theory.Subsequently,a statistical analysis of the causes was made by selecting 54 publicly available gas explosion case reports with detailed descriptions(after 2013,the website of Coal Supervision Department in various localities began to announce to the Society the full large and above gas explosion accident case report of the whole year,a total of 54 to 2016).Finally,the correlation of the cause factors of gas explosion accidents was analyzed based on the statistical analysis results.The main research work and conclusions are as follows:(1)The advantages,disadvantages and the applicable range of 10 kinds of accident causation models are compared and analyzed.The accident causation model tends to include human factors,physical factors,organizational factors and external factors;the path of the accident is developed from simple chain to system network.When using the systematic accident causation model,if the sufficiency is more valued,such kind non-cause classification models of STAMP(Systems Theoretic Accident Model and Processes)and FRAM(Functional Resonance Analysis Method)can be used to guarantee the depth and breadth of the analysis result,but the learning cost of these methods is high and there are some obstacles in the statistical analysis of multiple accidents.If efficiency is more valued,such kind cause classification models of CREAM(Cognitive Reliability and Error Analysis Method)and 24 Model can be applied,and these methods can be used for statistical analysis of multiple accidents.To meet the needs of the systematic accident causation analysis and statistical analysis,24 Model is chosen as the theoretical basis of the study.(2)A coal mine gas explosion accident cause classification system was established based on 24Model(24model-mine gas explosion,24model-mge),and accident analysis principles and methods were elaborated.The relationship between the causes,the objects of the accident,the consequences of the accident and the accident is expounded,and the object of this study is defined as an accident.The method of accident analysis was determined as follows:(1)extraction of proximate cause factors;(2)the factors causing the proximate causes are determined according to the classification module of 24MODEL-MGE,iteration by layer until all causes are finalized.(3)uses the counterfactual test(CT)and the causal sufficiency test(CST)are used to test the sufficiency and necessity of the extracted factors.Through the causation analysis of the gas explosion accidents,the feasibility and rationality of the 24 Model-MGE were confirmed.The feasibility of analyzing the gas explosion accidents with the 24Model-MGE is verified by the analysis of the single accident.The feasibility of multiple gas explosion accident statistics with 24 Model is verified by the statistical analysis of 54 accidents.(3)The causes of unsafe acts,individual factors,unsafe physical conditions,safety management system,safety culture and external factors in 54 gas explosion accidents were analyzed via 24MODEL-MGE.a)The unsafe acts causes of the gas explosion accident was explored,and the list of unsafe acts causes by various functional departments was given separately.The results show that the ratio to all unsafe acts(hereinafter referred to as ratio)of unsafe acts issued by management level is the highest,which is 54.20% and is the key factors to be improved in the unsafe acts causes.In the main production department,unsafe acts issued by the mining section,the excavation section,the ventilation section and safety section account for 8.29%,3.52%,16.58%,8.6% respectively,the total is 36.99%.Unsafe acts issued by other departments are relatively low,with 1.55% for the mechanical and electrical section,0.31% for the transport section,4.97% for the production technology section,1.35% for the production control center,0.41% for the equipment center,and 0.21% for the finance section.b)The individual factors causes of the gas explosion accident were explored.The results indicate that the key points of individual factors correction in management level are safety awareness,safety habits and safety psychology.In the main production departments,the individual factors correction of the coal mining section,the digging section and the ventilation section are focused on the safety knowledge,safety habits and safety psychology;The individual factors correction of the safety section is focused on safety habits and safety psychology.In other departments,the individual factors correction of electromechanical section is focused on safety knowledge,safety habits and safety psychology;The key points of the individual factors correction in the transport section are safety awareness and safety psychology;The key points of the individual factors correction in the production technology section is the safety psychology.The individual factors in the production control center,equipment center and finance section have a low frequency,which are no regularity.Through the causation analysis of the gas explosion accidents,the feasibility and rationality of the 24 Model-MGE were confirmed.The feasibility of analyzing the gas explosion accidents with the 24Model-MGE is verified by the analysis of the single accident.The feasibility of multiple gas explosion accident statistics with 24 Model is verified by the statistical analysis of 54 accidents.c)The unsafe physical conditions causes of the gas explosion were explored,and the list of unsafe physical conditions causes was given.The key points of the unsafe physical conditions prevention in the gas explosion accident are the unreasonable& unreliable ventilation system(27 times),the ventilator is insufficient(19 times),methane emission from goaf(12 times)in the gas accumulation;Spark(19 times)and firecracker spark(14 times)in the ignition source.The insufficient "six major systems" for safe haven(30 times)in the technical facilities;No safe production conditions(7 times)in the environmental conditions.d)The safety management system causes of the gas explosion accident were studied,and the list of deficiencies in the safety management system was given.The results show that the inspection(140 times),implementation and operation(137 times)are the key points to improve the safety management system in the phase of gas explosion accident prevention.The high frequency causes in the examination are the incident investigation,correction of nonconformity and preventive measures(98 times);compliance evaluation(38 times).The high frequency causes in implementation and operation are resource,function,responsibility,responsibility and authority(62 times),ability,training and consciousness(36),operation control(23 times).In the remaining safety management system causes,the planning appeared 34 times,and the high frequency causes in planning is the hazards identification,the risk assessment and the prevention measure determination(20 times).Management review and safety policy appeared 25 times,1 times respectively.e)The safety culture causes of the gas explosion accident were explored.The results show that the safety commitment(201 times),the safety responsibility(171 times)and the quality of the safety management system quality(146 times)appear the highest frequency,which are the key to improve the safety culture in the gas explosion accident prevention.The awareness of safety practice activities(103 times),the role of safety agencies(99 times),and the safety participation degree(54 times)are targeted guidance for specific aspects of corporate safety management,and the consolidation of these three types of safety culture principles reinforces the accident prevention.f)The external factors causes of the gas explosion accident were studied,and the list of external factors was given.The results show that the regulatory authorities(480 times)is the key point of the external factors in the gas explosion prevention,among which the high frequency causes are the supervision the performance of the subordinate units(143 times),the supervision of safety production(79 times),and the supervision on the main responsibility of safety production(71 times).The high frequency causes factors of the superior unit(64 times)are ineffective supervision(15 times)and the technical management is not in place(10 times).The other(3 times)shows that the the intermediary agencies randomly issued the mine safety evaluation report(3 times).(4)The correlation between unsafe acts,individual factors,safety management systems,safety culture,regulatory authorities and accident severity were analyzed.The results show that the severity of gas explosion accidents is related to the four dimensions of unsafe acts,individual factors,safety culture and regulatory authorities.Among them,there are 16,5,5,and 3 key cause factors in the dimensions of unsafe acts,safety culture,supervision,and individual factors respectively,which are related to the severity of gas explosion accidents.(5)The correlation between individual factors,safety management system,safety culture,regulatory authorities and unsafe acts were analyzed.The results show that the unsafe acts causes of the explosion accident are related to four dimensions of safety culture,regulatory authorities,safety management system and individual factor.Among them,there are 14,8,5 and 4 key cause factors in the dimensions of safety culture,regulatory authorities,safety management system and individual factors respectively,which are related to the severity of gas explosion accidents.(6)The prevention of gas explosion accident is a systematic project,which needs to establish comprehensive measures.The causes of the gas explosion cover human factors(unsafe acts and individual factors),physical factors(unsafe physical conditions),organizational factors(deficiencies in safety management systems and deficiencies in safety culture),and external factors.Therefore,in the gas explosion accidents prevention,it is necessary to conduct overall control from the above factors.
