Study On The Role Of Dyslipidemia In The Pathogenesis Of Diabetic Retinopathy

Objective: To investigate the relationship of triglyceride(TG),total cholesterol(TC),low-density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C),apolipoprotein AI(ApoAI),apolipoprotein B100(ApoB100)and other factorss with the occurs anddevelopment of diabetic retinopathy(diabetic retinopathy,DR),and initially reveals pathogenesis of DR.Methods: A retrospective cross-sectional study.Since June 2013 to December2015,elects patients from ophthalmology clinic in First People's Hospital,Shanghai Jiaotong University and subjects from Shanghai Si Jing District Health Center healthy total of 3799 cases,collected clinical data by sex,age,BMI,systolic blood pressure(SBP),diastolic blood pressure(DBP),pulse pressure,waist circumference(WC),hip circumference(HC)and recorded the general condition and medical history,collected laboratory biochemical indexes such as TG,TC,HDL,LDL,apoA,apoB,fasting blood glucose(FBG),2-hour postprandial glucose(2h PPG),glycosylated hemoglobinA1c(HbA1c),glycated albumin(GA-L).Application Elisa assay to detect angiopoietin-like protein 1(ANGPTL1),angiopoietin-like protein 2(ANGPTL2).Based on the results of laboratory tests and fundus photography of patients,they were divided into four groups,normal group(Normal group 1297 cases),with diabetes without diabetic retinopathy(DM no DR group 1002 cases),non-proliferative diabetic retinopathy(NPDR group,1280 cases)and proliferative diabetic retinopathy(PDR group,220 cases),the application of SPSS 21.0 statistical analysis system and logistics regression analysis.Results:(1)16 DR-related factors were analyzed,waist-to-hip ratio(WHR)was not statistically different among all groups.TG,TC,HDL-C,LDL-C,ApoA1,Apo B100,Apo AI / Apo B100,gender,BMI,SBP,DBP,FBG,2hPPG,HbAlC,GA-L and other factors have statistically significant.(2)Univariate logistics regression analysis shows that TC,HDL-C,Apo AI,Apo B100,Apo AI / Apo B100,SBP,DBP,FBG,2h PPG,HbA1 c,GA-L,BMI is a risk factor for DR.After adjusting for glucose,TG,TC,Apo AI,HbA1 c,GA-L,SBP are independent risk factors for the onset of DR.(3)The risk factors for proliferative retinopathy analysis found that the risk factors for proliferative DR include Apo AI,Apo B100,Apo AI / Apo B100,FBG,GA-L,BMI.(4)ANGPTL1 was negatively correlated with HDL-C,while positively with FBG and HbA1 c was.ANGPTL2 also negatively correlated with HDL-C,and positively correlated with FBG,2h PPG,HbA1 c,GA-L and LDL-C.ANGPTL1 and ANGPTL2 relate to occurrence and development of DR.Conclusion: TG,TC,HDL-C,LDL-C,ApoA1,Apo B100,Apo AI / Apo B100 are closely related toDR.Hyperglycemia and hyperlipemia interaction with each other to promote the development of DR.TG is probably independent risk factors for DR,HDL-C and Apo AI may be an independent protective factor for onset of DR.ANGPTL1 and ANGPTL2 relates to DR.
