The Influence Of Triglyeride And Total Cholesterol On Microalbuminuria Of Type 2 Diabetes Mellitus

Diabetic nephropathy (DN)is a serious complication of diabetes mellitus(DM).Microalbuminuria(MAU) is used in clinical to diagnose DN.There are so many clinical researches about the relationship between DN and MAU ,and it was found that elevated of plasma triglycerides(TG),small and dense low-density lipoprotein (LDL),decrease of high density lipoprotein cholesterol (HDL-C) is the main abnormal metabolism of serum lipids in patients with DN.Objective:To explore in the 2-type DM the relationship between abnormal TG,TC and renal damage in patients with DN.Method:Choose 86 patients ,all fulfilled the standards below :1. World Health Organization(WHO) Criteria for NIDDM in 1999.2.No DM acute complications .3.No infection.4.No renal damage before have DN.5.AST,ALT,TBIL,IBIL,DBIL,BUN,CRE,etc is't pathologic change.According to TG,TC,LDL-C and HDL-C,patients was divided into 2 groups(1)Dyslipidemia group, included 75 persons(59 persons TG>1.70mmol/L,59 persons TC>4.8mmol/L).⑵Non-dyslipidemia group, TG<1.80mmol/L,TC< 6.0mmol/L , LDL-C < 3.10mmol/L , including 11 persons.According to TG,TC and LDL-C divided the dyslipidemia group into 3 subgroups:A,TG group : TG >1.80mmol/L,TC<6.0mmol/L,including 28 persons,some of them LDL-C>3.10mmol/L;B,TC group: TG<1.80mmol/L,TC>6.0mmol/L or LDL-C>3.10mmol/L,including 17 patiens;C,TG+TC group:TG>1.80mmol/L,TC>6.0mmol/L,including 30 persons , some of them LDL-C > 3.10mmol/L.There are no significant difference among the 4 groups in DM course,hypertension(systolic pressure and diastolic pressure),GHbA1C (p>0.05).Result:1,The level of TG is evaluated significant in early stage of DN than TG's level reported in DM.2,MAU is evaluated significant when TG or LDL-C evaluated. 3,TG and LDL-C is positively correlate with MAU,TC is negatively correlate with MAU.Conclusion: The main abnormal metabolism of serum lipids is elevated of TG and LDL-C in the early stage of DN,it need to continue researching to identify if HDL-C is important to early stage of DN.The mechanism of abnormal metabolism of serum lipids induced DN is not very clear,but maybe the mechanism is: Oxidation reaction ;cytokin participating;the abnormality of cellular proliferation and apoptosis, increased extra cellular matrix (ECM).We should diagnose and therapy the early stage of DN rapidly and standard.
