Benign paroxysmal positional vertigo(BPPV),commonly known as otolith disease,is the most common peripheral vestibular disease.Although the effectivity of manipulative reduction is good,some patients have poor efficacy and are prone to relapse.Some patients still have discomforts such as heavy head,numbness of the scalp,and unstable walking et al.This discomfort is different from BPPV,called "residual symptoms." Traditional Chinese medicine is effective in improving the effectiveness of BPPV manipulative reduction,reducing the recurrence rate,and controlling the residual symptoms.Purpose:1.Make the systematic review and meta-analysis of randomized controlled trials of acupuncture combined with manipulative reduction in the treatment of BPPV evaluating the role of acupuncture in the treatment of BPPV.Make the systematic review and meta-analysis conducted on randomized controlled trials of the residual symptoms treated by TCM.The effectiveness and safety of TCM were evaluated.2.Observe the clinical effectiveness and satefy of Qing Xuan Xing Nao combined with manipulative reduction in the treatment of BPPV.Material and method:1.Search CNKI,VIP,Wanfang,Pubmed.The deadline is September 2017.Literature inclusion and exclusion criteria were established and all randomized controlled trials of acupuncture combined with manipulative reduction and randomized controlled trials of the residual symptoms treated by TCM were included.The Jadad score was used to evaluate the quality of the literature.The relevant literature was evaluated according to the CONSORT statement.The literature about acupuncture treatment was evaluated using STRICTA international standards.Meta-analysis was performed using computer software.Oddity ratios(RR values)were used to compare the enumeration data.The mean difference(MD)was used to compare the measurement date.According to the heterogeneity test,a fixed effect model or a random effect model was performed.2.According to inclusion criteria and exclusion criteria of the case,finally 60 patients entered the trial.The patients were divided into experimental group and control group randomly.Qing Xuan Xing Nao combined with reduction therapy was given to the experimental group,and placebo combined with reduction therapy was given to the control group.The effectiveness,recurrence rate,balance function and emotional state after treatment were compared.Follow-up was conducted in 1 month.Results:1.14 papers about the BPPV combined with acupuncture treatment were included.The Jadad score showed that most of the studies had lower scores.The CONSORT statement and STRICTA international standards were used to evaluate the included literature.The quality of the included literature needs to be improved,and the principle of randomization should be paid more attention,blindness,distribution and concealment need to be further strengthened.Meta-analysis showed that the effectiveness of the acupuncture combined with manipulative reduction was better than the control;TCM symptom scores was better in the acupuncture combined with manipulative reduction.TCM treatment about residual symptoms included in 16 articles,including 12 articles on TCM treatment and 4 articles on acupuncture treatment.The Jadad score showed that most studies scored low,and CONSORT for TCM Statement and STRICTA International Standard were applied.The quality of some of the studies included in the literature is low,and the sample size estimation,blinding method,and allocation concealment need to be further strengthened.Meta-analysis showed that there was no significant difference between the effectiveness of Chinese medicine in treatment of residual symptoms;the effectiveness of Chinese medicine in treatment of residual symptoms was better than that the control group and was superior than the blank control group;Chinese medicine in treatment of residual symptoms is better than the drug control group and blank control group in the TCM syndrome score scale.2.Sixty patients were included,including 30 males and 30 females.The mean age of the experimental group was 50.93±10.64,and the average age of the control group was 47.77±11.96.The effective rates at one week after treatment in experimental and control groups were respectively 96.67% and 86.67%,with no statistically significant difference.The DHI,HAMA,HDMA was better than the control group.For the recovery of balance function,the test group was also superior to the control group.The recurrence rate at the 1-month follow-up was lower in experimental and control groups were respectively 3.33% and 10%,with no statistically significant difference.Conclusion:1.The literature about acupuncture combined with manipulative reduction reported that acupuncture treatment can improve the effectiveness of manipulative reduction,reduce the recurrence rate of BPPV,improve the score of TCM symptoms,it is worthy of clinical promotion.2.The literature review of the traditional Chinese medicine treatment of residual symptoms shows that traditional Chinese medicine can effectively control the symptoms of residual symptoms,improve the DHI and TCM symptom scores,and reflect the traditional Chinese medicine's role in BPPV treatment.3.Qing Xuan Xing Nao can improve residual symptoms,control anxiety and depression,improve balance function recovery and so on.It has a positive effect on treatment.
