Evaluation of diagnostic efficacy of diabetic peripheral neuropathyPurpose: To evaluate the diagnostic value of stress perception,vibration perception,temperature perception,pain and ankle reflex in DPN,and to find an economical,simple,accurate and rapid method to diagnose DPN.Methods: The nerve conduction velocity were determined in 874 patients with type 2 diabetes,which from June 2015 to December 2016,and in the Affiliated Hospital of Liaoning University of Traditional Chinese Medicine Department of Endocrinology.Pressure sensation,vibration sensation,temperature sensation,pain and ankle reflexes were assaied simultaneously.Sensitivity,specificity,positive predictive value,negative predictive value,Jorden index,Kappa value(k value)and ROC curve were considered evaluating indicators to evaluate the diagnostic efficiency of 5 methods.Results: In the 5 methods,the sensitivity,specificity and K value of the ankle reflex were 58.25%,91.51%and 0.301 respectively,and compared with diagnostic criteria the consistency is the best.The sensitivity and consistency of stress perception were the worst.The sensitivity,specificity and K value of the combination of temperature and ankle reflexes were 76.02%,72.73% and 0.378 respectively,the consistency is the best.The sensitivity,specificity and K value of the combination of temperature sense + vibration and ankle reflex were 80.81%,67.27% and 0.404,respectively,its consistency is the best.Conclusion: The diagnostic efficiency of the combination of temperature sense + vibration and ankle reflex in the 5 methods is the best and it is a simple and economical DPN screening method.Effect of Low Frequency Electric Pulse Therapy Combined with Mudan Granule on the Patients with Diabetic Peripheral NeuropathyPurpose: To investigate the effect of low frequency electric pulse therapy combined with mudan granule on patients with diabetic peripheral neuropathy.Methods: 90 patients with diabetic peripheral neuropathy who were treated in our hospital were selected and randomly divided into the control group and the experimental group.The patients in the control group were treated with mecobalamin tablets,while the patients in the experimental group were treated with mudan granule and acupuncture point stimulation of low frequency pulse electric therapeutic instrument.Then the FBG,P2 h BG and Hb Alc levels of patients in the two groups were detected and compared before and after treatment.And the motor nerve conduction velocity and sensory nerve conduction velocity of the nerve in the common sense were compared by EMG.Results: Compared with before treatment,the common nerve and median nerve MCV and SCV were higher,the experimental group was more obviously than that of the control group,and the difference was statistically significant(P<0.05).The TCSS scores were lower,and the experimental group were more obviously than that of the control group,and the difference was statistically significant(P<0.05).Conclusion: Low frequency pulse electric therapeutic apparatus combined with Mudan granule could significantly improve the diabetic peripheral nerve lesions and more effective than mecobalamin tablets alone.
