Retrospective Analysis Of Treating Dry Eyes In Singapore Chung Hwa Medical Institution With TCM Syndrome Differentiation

Objective:To collect and sort out the dry eye medical records of the Ophthalmology Group of the Singapore Chung Hwa Medical Institution.From Jan 2013 to May 2019,a total of 1086 cases were new consultations.Through analysis of clinical data by means of data mining,different types of dry eye TCM syndromes were identified.The traditional Chinese medicines and prescriptions were analyzed and sorted to provide a more effective and comprehensive theoretical and practical basis for the clinical treatment of dry eye using traditional Chinese medicine in the future.Methods:The Medcase V3.2 data record mining system was used to enter the clinical cases of the Ophthalmology Group of Singapore Chung Hwa Medical Institution from Jan 2013 to May 2019.A database was established,and screening was performed in strict accordance with the inclusion and exclusion criteria.The frequency method,association rules,cluster analysis and other methods were applied to the 1086 medical cases(all of which were newly diagnosed)that met the approved criteria.The main focus undertaken was on the symptoms,pathogenesis,and prescriptions in the medical records.The mining results were analyzed,classified and summarized.Results:From Jan 2013 to May 2019,the gender distribution of dry eye patients in the Ophthalmology Group of Singapore Chung Hwa Medical Institution had more women than men.Of the 1086 cases,women accounted for the majority of dry eye patients,accounting for 74.85%or 813 cases,and men accounted for 25.15%or 273 cases.In terms of age distribution,the number of dry eye patients after the age of 40 accounted for 1016 cases of the total data of 1086 cases,amounting to about 93%of the total.Of the 1086 medical records,the main clinical dry eye symptoms are:dry eyes 83.42%,poor sleep 27.26%,itching 17.22%,tearing 16.11%,dry mouth 15.93%,blurred vision 15.01%,eye pain 13.26%and photophobia 10.13%.The main syndrome type distributions are liver kidney yin deficiency 24.13%,lung kidney yin deficiency 12.06%,qi stagnation blood stasis 11.14%,qi blood deficiency 9.02%,qi deficiency blood stasis 6.45%,qi yin deficiency 3.04%,liver spleen disorder 1.38%,yin deficiency 1.01%and blood stasis 0.92%.The distribution frequencies of the main prescriptions are Qi Ju Gan Lu Yin 65.75%,Zengye Tang 10.22%,Xiao Chai Hu Tang 3.31%,Sang Ju Yin 3.04%,Danzhi Xiaoyao San 2.76%,Wendan Tang 2.76%,Shengmai San 2.58%,Xiangsha Liujunzi Tang 2.58%,Qiju Dihuang Wan 2.21%,and Guipi Tang 1.75%.Including others there are 20 prescriptions.The distributions of the main Chinese medicines prescribed during the consultations were 90.7%of Juhua,88.86%of Maidong,88.21%of Shengdi,79.65%of Shihu,77.72%of Baihe,75.41%of Zhiqiao,72.84%of Chenpi,71.09%of Gouqizi,69.15%of Taizishen and 36.19%of Sangye.Including others there are 36 medicines.Conclusion:The incidence of dry eye patients increases with age and women have a higher risk of contracting dry eye condition than men of the same age.The main syndrome type distributions are liver kidney yin deficiency,lung kidney yin deficiency,qi blood deficiency,and qi stagnation blood stasis.The core drugs for dry eye treatment are Juhua,Maidong,Shengdi,Shihu,Baihe and Zhiqiao.Qi Ju Gan Lu Yin is the core prescription for the treatment of dry eyes.Its medical composition is Gouqizi,Maidong,Baihe,Shihu,Shengdi,Zhiqiao,Chenpi,Juhua and Taizishen.Qi Ju Gan Lu Yin with Zengye Tang and Sang Ju Yin act as overlapping complementary prescriptions.They are being used to treat dry eye syndrome types of yin deficiency fever and fluid loss.
