In order to understand the pathological change and seek for the pathogenesis of bronchoalveolar lavage fluid (BALF) and lung of young rabbits suffered from aspiration pneumonia (AP)by duodenogastroesophageal reflux (DGER). The result of it may be helpful to the diagnosis and the treatment to DGER in clinical medicine. The method was to injected gestric fluid and bile into the trachea of young rabbits in experiment group, the animal model of aspiration pneumonia by DGER made. Tested the BALF. observed the pathological tissue of the lung, and compareed it with control ~-oup after 2,8,24,72 hours. The changes in experiment group were as follow: (1 )The number of cells in BALF increase, the type of cells are both the alveolar macrophage (AM) and polvmorphonuclear leukoc~tes (PMN) in majority, the highest peak of the PM7N at 8 hour. (2) The total protein in BALF increases. (3) The tunmor necrosis factor a (TNF a ) and the interleukin-8 (IL-8) in BALF increase obviously.(4)The lung body index increases. (5) The findings of pathology indicate that inflammatory cells invade the pulmonary alveolus, the copillarv was dilatant and congestive.(6)The findings of ultrastractureal pathology indicate that pneumocyte (PC) and vascular endothelial cells (VEC) degenerattion and necrosis, lamellar bodies (LB) in PC type II are vacuolation. The study confirms that the gestric fluid and the bile are both attact factors in AP by DGER. It caused injury either direct or indirect. This study is the first observate pathological changes of cell number. cell type. total protein and cytokine (TNF a IL-8) in BALF and lung suffered from AP by DGER. AM and PMN are the main inflammatory cells In it, TNF a and IL-8are inflammatory mediators in this response. The vacuolation in LB can reduce the pulmonary surfactant.the degeneration and the necrosis in PC and VEC can increase the permeability of alveolar capillary membrane.
