Objective To analyze the pathogenesis in patients with acute lacunar infarction(ALI), which caused by small vessel disease with imaging to diagnosis the SVD in the early time.Methods According to TOAST classification, excluding cardiac and large artery atherosclerotic embolic etiology, there were106patients with ALI,their clinical data and imaging findings were analyzed, and according to Fazekas criteria, patients were divided into two groups: mild white matter lesions (grade0and grade1) and severe white matter lesions (grade2and grade3), their clinical information and imaging characteristics were analyzed.Results The patients who were older than50years and have hypertension were more common in the patients of ALI; Perivascular spaces (PVS)and white matter lesions (WML)are common MR imaging findings in patients with ALI.The incidence of asymptomatic cerebral infarctionsevere (SCI) and hypertension was higher in serious cerebral small vascular disease(SVD).Conclusions People older than50years old and with hypertension are closely associated with ALI which is caused by SVD, it will be useful to prevent SVD through control blood pressure in early time; MR imaging findings in patients with ALI contained SCI and white matter lesions, suggesting that they are may be heavier degree of cerebral small vessel disease.
