Analysis Of The Prescription Of Drug - Induced Nephrotic Proteinuria And Its Curative Effect

This article included two parts:the literature view and re-trospective study.Two literature views were the understanding of the proternuria in the angle of western medicine and traditional Chinese medicine, the use of data dining in the TCM research. The mechanism of the generating, damaging and the treatment were summarized in western understanding, and the pathogenesis, syndrome differentiation and treatment in TCM view. The methods and function of data dining which was widely used were analyzed in the second literature view.The second part was a clinical study:Objective:By means of collecting cases, data entry and analysis, the study was aimed at concluding the individual treatment experience of chronic kidney disease with proteinuria, exploring medication regularities with data mining technique and analyzing the efficacy of Chinese medicine.Method:1. Collected the outpatients’ records from Professor Du of Chi-na-Japan Friendship Hospital. The TCM inheritance support system was used to input information and process data. The common used herbs of the total patients and the ones using hormone or thunder god vine were summarized by the frequency, the core combinations of drugs were analyzed by revising mutual information, the new prescriptions were concluded by using complex system entropy cluster and unsupervised hierarchical cluster.2. Followed up 24-h urinary protein and grades of symptoms, then got the statistics and analysis by using SPSS 17.0.Results:1. Based on the 58 patients’427 prescriptions, the common symptoms were edema, fatigue and tired, low back pain, dizzy, tinnitus, chilly, ocular dryness, weak breath, laziness to speak and numbness, sore swollen throat, dark red tongue, thin and white fur, deep and slippery pulse were the most frequently. Analyzed the TCM Syndrome Types, spleen and kidney Qi deficiency and damp, blood stasis, damp-heat syndrome were common. In the 194 herbs used, the top ten were poria cocos, rhizome alismatis, herba lycopi, astragalus mongholicus, grifola, rhizome attactylodis macrocephalae, rehmannia, moutan bark, cornus, dandelion, rhizome dioscoreae. The herb can class as tonic herbs, diuresis detumescence, blood-regulating,clearing damp-hest, clearing heat a detoxication,astringent medicine. The prescriptions were formed 12-15 herbs with dose of 12-30g2. The symptoms of patients who used hormone were same as the commo the ones used thunder god vine did not have edema. Poria cocos, rhizo alismatis, grifola, rehmannia, herba lycopi, the root bark of the peo tree, teasel, dandelion were the key herbs.3. Analyzed the regulation of prescriptions using association rul algorithm, then got the 67 core conclusions which contain 3 to 5 herb and 7 new developed prescriptions based on the complex system entro cluster analysis.Referred to the clinical experienced new prescriptions we summarized as follow:treats, chicao, fried atractylodes,anemarrhen moutan; scutellaria, pinellia ternate, radix bupleuri; dodder, tease dandelion, fructus psoraleae, focal gardenia; fructus psoralea donkey-hide gelatin, teasel,radix glycyrrhizae preparata; rehma nia, grifola,cornus, donkey-hide gelatin, moutan bark, rhizoma diosc reae,rhizoma anemarrhenae,largehead atractylodes.4. There are significant differences in urine protein before a after the treatment (P<0.05), the total effective rate was 73.91%, t groups with different proteinuria had no statistical differences. T treatment could work in the fist three months. The effective rate syndrome was 86.21 percent.Conclusion:1. Based on the data raining technology, we can get the regulari of prescription and new ones. It played more and more important ro in the conclusion and inheritance of rich experience doctors.2. TCM had obvious effect in the treatment of CKD patients who h urine protein, no matter whether or not to use hormone and immu inhibitors, and it can take effect in three months.
