Clinical Study On Treatment Of Postoperative Hemorrhagic Anemia With Modified Danggui Buxue Decoction In Perioperative Period Of Artificial Total Knee Replacement

Objective:The present study explores the short-term clinical efficacy of Modified Dangguibuxue on treating hemorrhagic anemia during the perioperation period of total knee arthroplasty and investigates the relationship between the postoperative functional recovery and the pain relief of knee joint.Methods:During the period from February 2014 to February 2015, sixty patients were treated with unilateral total knee arthroplasty who had knee osteoarthritis for the first time in Wangjing Hospital, China. By using the method of the random number table (randomized single-blind), the selected patients were divided into two groups, the treatment group and the control group. There were 30 people in each group. The treatment group was treated with Modified Dangguibuxue (Huangqi 30g, Danggui 6g, Fuling 12g, Baizhu 12g), which was taken with 300 ml water. The patients were asked to drink it twice a day and they began to drink it three days before their surgery. And from the first day to the seventh day after surgery, the control group of perioperation period was treated without Modified Dangguibuxue which was treated with the same routine treatment with treatment group. On the third day before operation (before treatment), the first day after the operation, the seventh day after the operation, VAS pain score was examined, as well as HGB, GCT. On the third day before operation, the seventh day after the operation, the fourteenth day after the operation, the HSS score system and the Visual Analog Scale were both used to respectively analyze the knee joint function in perioperative and the degree of their pain. Results:1 Hemoglobin:Three days before operation (before treatment), the treatment group (138.33 ± 8.42) g/L, the control group (138.20±8.44) g/L, between the two groups, the difference was not significant (P>0.05);first day after the operation, treatment group (103.00±7.02) g/L, the control group (95.17±5.24) g/L, the number of two groups were decreased significantly, but the treatment group decreased lower than that in control group, two groups with significant difference statistically (P<0.05); third days after operation, the treatment group (114.60±7.59) g/L, the control group (109.57±8.96) g/L seventh days after operation, the treatment group (125.13±8.42) g/L, the control group (115.40±7.67) g/L, the date of hemoglobin were increased, but the treatment group was higher than the control group, between two groups, the significant difference was statistical (P<<0.05).2 Hematocrit:3 days before operation (before treatment), treatment group (42.00 ±2.51)%,the control group (42.07±2.45)%, between the two groups, no significant difference (P>0.05);first day after the operation, treatment group (34.70±2.17)%, the control group (30.50±1.55)%, the number of two groups were decreased significantly, but the treatment group decreased lower than that in control group, two groups with significant difference statistically (P<0.05); third days after operation, the treatment group (40.03±2.59)%, the control group (34.07±2.29)%, seventh days after operation, the treatment group (42.00±2.33)%, the control group (38.50±2.39)%, the date of hemoglobin were increased, but the treatment group was higher than the control group, between two groups, the significant difference was statistical(P<0.05).3 HSS knee function scores:Three days before the operation, the treatment group gained (62.20±4.70) points. No significant difference existed (P>0.05). Seven days after the operation, the control group gained (68.10±4.68) points while the control group gained (63.57±5.03) points. The results were significantly different from each other. Fourteen days after the operation gained (81.97± 4.56) points while the control group gained (74.67±4.44) points. At the same time, the difference was significant with statistical significance(P<0.05).4 VAS pain scores:Three days before the operation, the treatment group gained (7.17±1.42) points while the control group gained (7.30±1.49) points. No significant difference was found between groups (P>0.05). Seven days after the operation, the treatment group gained (4.23±0.86) pointes while the control group gained (4.17±0.95) points. Fourteenth days after the operation:the treatment group gained (2.17±0.70) points while the control group gained (2.13 ±0.68) points. The comparison results of the same observation period did not show any statistically significant difference between groups (P>0.05).During this study, both the groups had no obvious hemorrhagic events and some other related complications. Conclusion:Both in the treatment group and the control group, the patients with hemorrhagic anemia have been better after the operation. Total knee arthroplasty could help them restore the function of knee joint and relieve their pain. The usage of Dangguibuxue Modified greatly contributes to curative effects on the patients with hemorrhagic anemia. It helps the patients recovery postoperative joint function but cannot relieve their pain.
