Background:With the aging of the population and the change of the working environment,The knee osteoarthritis has become one of the common diseases that endanger the health of the elderly after hypertension and diabetes. Most of the knee cartilage degeneration is limited to the medial compartment of the knee joint,In the end of the medial knee osteoarthritis, the choice of total knee replacement(TKA)or the Unicompartment Knee Arthroplasty(UKA)is still a significant controversy.Total knee arthroplasty has been highly praised for its Experienced surgical techniques and excellent long-term survival rate. The early stage of unicompartment knee arthroplasty because of failure rate and repair rate is high is not be welcome. With continuous improvement of the indications, surgical technology continues to mature, prosthesis design of continuous improvement and innovation, especially the minimally invasive unicompartmental knee arthroplasty for osteoarthritis than total knee arthroplasty performed a very low mortality rate, intraoperative and postoperative low complications, faster speed of recovery, less bleeding quantity, better proprioception, shorter hospitalization time and other advantages. Objective:To compare the early clinical efficacy of minimally invasive unicompartmental knee arthroplasty and total knee arthroplasty for the treatment of end stage osteoarthritis of the medial compartment of the knee joint. Methods:From January 2013 to January 2015,Selected Patients due to unilateral knee medial compartment osteoarthritis for knee arthroplasty patients in the orthopedic department of our hospital?Use SPSS 17.0 statistical software Compared the KSS scorein the unicompartmental knee arthroplasty group and total knee arthroplasty group ?KSS score in the 1 year after operation?FJS score after 1 years of operation?Operative time?time of stay?number of blood loss?Postoperative drainage volume?blood transfusion rate?incision and joint infection rate?number of pulmonary embolism and deep vein thrombosis, and the number of revision, and compare with the KSS score of 1years before and after operation in the unicompartmental knee arthroplasty,KSS score of total knee arthroplasty group and 1 year after operation,Evaluation of early clinical efficacy of unicompartment knee ahroplasty and total knee arthroplasty in the treatment of ending stage knee medial compartment osteoarthritis. Results:After statistical analysis, there was no significant difference in KSS scores between the two groups before operation;,the KSS scores After 1 years of operation were significantly improved compared with the preoperative in the two groups, the different has significant difference; The UKA group of KSS score after 1 year postoperation was higher than the TKA group; The FJS score of the UKA group was higher than the TKA group after 1 year postoperation; The operation time of UKA group was significantly shorter than that of TKA group; The amount quantity of blood loss, drainage, blood transfusion, hospital time, deep venous thrombosis and pulmonary embolism rate of the UKA group were significantly lower than the TKA group; After 1year follow-up period two groups were no patient implants dislocation or loosening and subsidence, periprosthetic fracture, polyethylene liner dislocation, knee joint lateral tibiofemoral compartment and patellafemoral room appear progressive osteoarthritis need revision cases; There was no infection in the joint and the incision between the two groups? Conclusion:Total knee arthroplasty and minimal invasive unicompartmental knee arthroplasty all have good early clinical effect in end stage knee osteoarthritis of lesion limited in the medial compartment of the knee, can alleviate pain, restore good function, meet the needs of daily life, improve the quality of life.Even more,Minimally invasive unicompartmental knee arthroplasty,in which the normal physiological conjective were retained, and the anterior and anterior cruciate ligament and bone mass werepreserved,Postoperation Higher knee function score and patient satisfaction will got,Meanwhile, the quantity of blood loss is litter?soft tissue injury is less?bed time is short, have little trauma,To avoid the occurrence of many postoperative complications.
