Research On The Evaluation Index System Of Chinese Community Medical Service Ability

It is an inevitable requirement for evaluating community TCM service capabilities scientifically that to build a reasonable and effective evaluating indexes system for community TCM service capabilities. Community TCM service is the entry point of this paper, as well as determination on a basis of the analysis of conventional method of community health service evaluation. As the result, we come up with the suggestions on building a evaluating indexes system. To realize the current progress of community TCM developing situations, service effectiveness and the existing problems objectively.Furthermore, to propose appropriate measures for corresponding improvement to maximize the capabilities of community TCM services.[Objective]To give a comprehensive evaluation on community TCM service capabilities, as well as lay the foundation for scientific, standardized and normalized management of community TCM service by building an evaluation indexes system which is general, scientific and fit for the current community TCM service. With the key point of community TCM service, the evaluation system contains four aspects which are personnel, technology, service and management.[Methods](1)Literature Research. According to the reality of community TCM service, looked up a number of related policies and documents published both domestic and overseas. According to the given principles, collected indexes related to community TCM service as much as possible to build the primary indexes database. (2)Focus Group Discussion. Determine the framework which includes personnel, technique, service and management to draw up the evaluation indexes system of community TCM service capabilities. (3)Delphi Method. Weighted scoring the importance and the operability by a two-round consulting to the selected experts from areas of health service management and community TCM management. According to the suggestions for revision and criterion proposed by experts, build the evaluation indexes system of community TCM service capabilities. (4) Analytic Hierarchy Process. Under the evaluation indexes system, judgment matrixes have been formed. Experts take every level indexes for each pairwise comparison. Calculating weight coefficients through AHP software and conduct consistency check. (5)Questionnaire Survey. Take 10 community health service centers as survey respondents by the method of random sampling.Collect all of the necessary data from questionnaire survey. Take reliability and validity test by Cronbach’s a coefficient and factor analysis. Evaluate community TCM service capabilities through comprehensive evaluation method.[Results] (1) Experts participating in the formulation of evaluation indexes system:eight experts from areas of health service management and community TCM management; the available response rate of two round inquiry was both hundred percent; Expert group authority coefficient value of 0.83; Therefore, experts from the higher positive degree and extent of authority, index selection is reasonable. (2) Composition of the evaluation indexes system:Establish an evaluation indexes system of community TCM service capabilities, which contains 4 first grade indexes,12 second grade indexes and 37 third grade indexes. They give a comprehensive evaluation of community TCM service capabilities. (3) Weighting of the evaluation indexes system:Application-level analysis build a comparison matrix used to determine the weight of root and the largest eigenvalue and the level of single-sort, sort of consistency in the level of total test The results showed that consistency at all levels to meet the requirements of indicators in order to work out the value of an index weight are:0.222,0.458,0.119,0.201. And use the principle of multiplication to calculate the probability of indicators at all levels of the portfolio weights. (4) The evaluation index system of reliability and validity:The internal reliability of indicators is reflected by calculation of Cronbach’s a coefficient. The results showed that standardized Cronbach’s a coefficient is 0.777. Factor analysis is made to test the structural validity of the indicator system. The results showed that totally three common factors were extracted, the accumulative variance reached 91.428%. The index system has good reliability and validity.[Conclusions] (1) Delphi expert consultation of experts positive degree and the degree of authority is relatively high. Therefore, the prediction precision of study was relative high and the outcome was reliable and effective. (2) The study on community personnel, technology, service and management of TCM four aspects to build the evaluation index system that is broad. It has a more reasonable structure and a complete content and index has a certain scientific nature and operability. (3) On the basis of judgment by experts, the weighting of indicators is calculated with Application-level analysis to ensure scientific, accurate and rationale calculation results. (4)The evaluation indexes system can meet their needs to reflect the current situation of community TCM service capabilities as well as provide a comprehensive and detailed reference to the assessment criteria of community TCM service capabilities.
