Clinical Study On Treatment Of Acute Radiation Proctitis With Retention Enema With

Radiation therapy is an important means in the treatment of abdominal and pelvic malignant tumors.Radiation proctitis is a common complication of pelvic tumor patient radiotherapy. Main symptoms of Radiation proctitis include abdominal pain, nausea, vomiting, diarrhoea, discharge mucus or blood, tenesmus and so on. Mucosal edema with congestive, Erosion,bleeding, Ulcer formation, Mucosal thickening, Lumen stenosis were seen in the view of Rectal Speculum. Along with the increasing radiation treatment of pelvic malignant tumors, there is an upward trend in the incidence of radiation proctitis.The main purpose of radioactive rectitis treated is to alleviate symptoms, alleviate the suffering patients. A variety of drugs are used for treatment of radiation proctitis, which include intestinal mucosal protective agents, mucosal barrier injury repair agents,cell protection agents, somafostatin,parenteral nutrition. Although some effect has been maken by the drugs, there is no specific medicine or method on the treatment.Traditional Chinese Medicine has gathered ample clinical experience on the treatment of carcinoma, owned advanced technical technology,and were approved by many doctors and patients, gradually became one of the valuable ways in treating the malignant carcinoma. TCM retention enema is an important part of traditional Chinese medicine,which has many advantanges such as easy to perform;few side effects;rapid onset;avoiding gastroinestinal irritation;whithout resistance and addiction,etc. Chinese medicine powder for safe treatment of Acute radiation proctitis. The treatment of Pingkuisan Powder on acute radiation proctitis, can significantly improve the patient’s clinical symptoms, and less adverse reactions, significantly better than western medicine alone.This study used randomized controlled methods,the clinical manifestations were classificated according to TCM sympotom score,combined with colonscopy. It could be benefited form this method for in-depth study of the treatment of radiation proctitis.And the theory of treatment of radiation damage by TCM was further improved and perfected. Objective:To observe the therapeutic effect of Pingkuisan Powder on Acute radiation proctitis,explore the effect of Pingkuisan Powder on alleviating pain in the low abdomen;defecation difficulties;loose stool;pus and blood stool;tenesmus,imporoving qulity of life.Methods:Complying with evidence-based medicine, This study adopted randomized control trials and prospective clinical research method, in the light of some diagnostic、included、exclude standards, the enrolled hospitalized patients were randomly divided into two groups, treatment group and control group. In these two groups, The treatment group of30patients Pingkuisan (Xi’an Jiaotong University Pharmaceutical Co., Ltd.) retention enema; The control group of30patients with gentamicin, lidocaine injection, dexamethasone, saline mixed retention enema. Both groups were observed after14days of treatment. Observe and compare the Treatment efficacy, Quality of Life, Clinical symptom score, Proctoscopy grade and adverse reaction of the two groups.Results:A total of60patients were enrolled in this study from2010.10to2012.10, with treatment group30cases, control group30cases. In the treatment g roup,27cases were efficient(90.0%)and control group22cases(73.3%).There wa s evident difference (P<0.05) in efficiency between Pingkuisan Powder and rout ine western medicine. And significant difference (P<0.05, P<0.01)was observed in alleviating pain in the low abdomen;defecation difficulties;loose stool;pus and blood stool;tenesmus, treatment group worked obviously efficiently than contro1group; Both of their KPS scores were increased, but without evident differen ce (p>0.05); In comparison of rectal mucosal injury grade between the two gro ups,there was evident difference (P<0.05).Two groups of patients during treatme nt had no obvious adverse reaction.Conclusion:In the clinical curative effect of acute radiation proctitis and rectal mucosal injury grade, Pingkuisan Powder was more effective than routine western medicine. Both treatment methods had no obvious side effects.
