Treatment Of Osteoarthritis With Osteoarthritis Of The Knee

Objective:To explore the clinical effects of knee arthroplasty with Traditional Chinese Medicine Zhuanggupian for osteoarthritis with osteoporosis. Methods: Patients were diagnosed by dual-energy X-ray absorptiometry (DEXA) of osteoporosis (T<2.5SD) with knee arthroplasty in patients with osteoarthritis and osteoporosis from October2011to August2013.69patients were randomly divided into experimental group, control group and blank group,23cases respectively.63TKA,5UKA,1individulized knee arthroplasty, aging54-80, an average of69.5years old, body mass index (BMI)21-30. All surgeries were performed by the same chief physician. The patients of experimental group treated with Zhuanggupian and calcium carbonate D3;control group treated with calcium carbonate D3only; blank group with placebo for3months. The clinical symptoms and degrees of pain relief, bone mineral density (BMD)were evaluated. HSS scores were recorded at the last following up. Results:Fall off:4cases,1case of test group,2cases of control group,1case of blank group, followed up of6-28months, an average of20.4months, no infection,2cases of joint pain, maybe associated with intra-articular synovial plica hyperplasia, the rest are satisfied results. The3groups have no significant difference compared with the preoperative HSS scores. Compared with the control group and test group, the patients postoperative HSS scores was statistically significant which is higher than that of the control group. Compared with BMD of3group, P>0.05, there was no statistically significant difference. While the experimental group and control group compared with the blank group, BMD (P<0.05), and the experimental group is higher than the control group. Preoperative and postoperative1month treatment group and control group compared with the blank group blood level Ca, P, ALP, P>0.05. But Ca, ALP tested after3months, P<0.05, P tested P>0.05. Conclusion:The knee arthroplasty with Zhuanggupian for patients with osteoarthritis and osteoporosis is effective. It can improve BMD and the changes of serum Ca, ALP. It may lead to osteoblast activity, promote bone formation, but the specific mechanism needs to be explored.
