Objective:Through a comparative study of the early clinical outcome of oxford unicompartmental knee arthroplasty(UKA)and total knee arthroplasty(TKA)for the treatment of medial compartmental knee osteoarthritis,to investigate the clinical advantages of oxford unicompartmental knee arthroplasty.Methods:Selected 76 cases of patients with medial compartmental knee osteoarthritis who admitted to hospital from Orthopaedics Department of Traditional Chinese Medicine of Jiangsu Province from October 2015 to January 2017,and recorded the maximum knee flexion,HSS score and WOMAC score.The UKA group contained 38 patients,18 males(18 knees)and 20 females(20 knees);The TKA group contained 10 patients,19 males(19 knees),and 19 females(19 knees).All the operation were conducted by the same operator after the relevant preoperative examinations were performed.The operative time,intraoperative blood loss,and postoperative drainage were recorded.The hemoglobin level was reviewed and recorded on the fourth postoperative day.The maximum knee flexion,HSS scores and WOMAC scores were assessed at follow-up at 3 months,6 months,and 12 months after surgery.The evaluation of two groups focued on the operative time,blood loss,the decreased levels of postoperative hemoglobin,the maximum knee flexion,HSS score,and WOMAC score.Results:Two groups recepted satisfactory clinical outcome.There were no complications such as deep venous thrombosis,prosthesis loosening,periprosthetic fractures,and infections.The follow-up results showed that the operation time of UKA group was 40.9±4.137min,which was significantly lower than that of TKA group 57.2±4.442min;the decrease of hemoglobin in UKA group was 16.2±3.667g/L,which was significantly less than that of TKA group 28.0±3.432g/L;the amount of bleeding in UKA group was 150.0±55.902ml,which was significantly less than that of TKA group 350.0±66.667ml;Preoperative maximum knee flexion in two group had no significant difference,At 3 months,6 months and 12 months after operation,the knee flexion in the UKA group was 101.1±6.509°,111.2±6.972°and 120.0±6.124°,which was significantly greater than that in the TKA group(95.0±5.270°,100.0±8.165°and 105.4±6.236°).Preoperative HSS scores and WOMAC scores in two group had no significant difference,At 3 months,6 months and 12 months after operation,the HSS scores in the UKA group was 79.4±5.548,88.0±4.610 and 91.6±2.784,which was significantly higher than that in the TKA group at the same time.At 3 months,6 months and 12 months after operation,the WOMAC scores in the UKA group was 46.0±3.428,26.2±3.969 and 10.0±3.202,which was significantly lower than that in the.TKA group at the same time.Conclusion:Compared with TKA,UKA has the advantages of short operation time,less trauma,less bleeding,fewer complications,better proprioception,and faster postoperative functional recovery.It has a significant clinical effect.in the treatment of medial compartmental knee osteoarthritis.
