Comparative Analysis Of The Efficacy Of PFNA And External Fixation In The Treatment Of Intertrochanteric Fractures In The Elderly

Objective:We objective analysed the clinical curative effect of two kinds of operation mode and discussed their characteristics by clinical observation of PFNA and external fixationin elderly patients with femoral intertrochanteric fracture so that to provide reference for clinicalselection of suitable operation method of intertro- chanteric fracture in the treatment of senile femoral.Method:By using systematic review method,we collected 74 patients with femoral intertrochanteric fracture who treated in Seven Department of bone in Foshan Hospital of TCM from 2013 April to 2014 November of which 34 patients using PFNA treatment, 40 cases were treated with external fixation.We observed and analysed operation time,bleeding volume during operation,hospitalization time, clinical healing time of fracture of two groups of patients and then made statistic analysis.Result:There were significant difference between external fixation and PFNA groups(P<0.05)on operation time(35.20±5.11vs42.79±4.66min),the amount of bleeding during operation(4.95±1.06vs122.35±12.81ml),the time of hospitalization(8.07±2.15 vs 11.38±2.57day),External fixation group had shorter operation time, less bleeding, shorter hospitalization time.The two groups in the clinical healing time of fracture(11.00±1.24vs10.68±1.57week) has no obvious difference(P>0.05). Postoperative complications in PFNA group is better than the external fixator group.Harriss score after hip jointtrauma was no significant difference between two groups(P>0.05).Conclusion:PFNA and external fixator have good effectin the treatment of intertro- chanteric fracture of femur in the elderly.External fixation has the advantages of simple operation,less trauma,less bleeding.So that it is specially suitable for elderly people who are accompanied with severe medical condition and have poor tolerance of operation.Although it would bepin tract infection,knee joint stiffness and other complications,they could generally restore after treatment.So it is a good method currently treating senile femoral intertrochanteric fracture,and it is worthy of clinical promotion.
