Objective Detection the distribution characteristics of specific allergen for patients with bronchial asthma at coastal area of Shandong Province and investigate the association between gene CD14 promoter SNPs with serum total-Ig E and eosinophil levels in atopic asthma and non-atopic asthma.Methods Total 16 common allergens in 2152 patients with bronchial asthma clinical patients were detected in vitro by automatic allergens quantitative detection system(Sweden Phadia 250). Analysis of various allergens positive rate in 1850 patients with asthma and compare the difference in different age segments of the allergen distribution. We selected 100 cases of atopic asthma(n=100) and 52 cases of non-atopic asthma(n=52) for further study. Six CD14 gene SNPs were analyzed using PCR and gene sequencing.Both serum total-Ig E and eosinophil levels were detected at the same time. The association was evaluated between genotype frequencies of CD14 gene loci with total-Ig E and eosinophil levels in atopic asthma and non-atopic asthma.Results The positive percentage of bronchial asthma with common specific allergens is about 86% and specific allergens mainly are dust mites(83.2%) and house dust mites(81.7%) which with significant difference(P<0.01). In the analysis of age stages,the positive rate of fish is higher in the elderly group(24.7%) compared with other allergens which has significant difference(P<0.05). In non-atopic asthma, total-Ig E levels of the heterozygous genotypes were significant higher than that of the homozygous genotypes in CD14-260、CD14-651、CD14-911 和 CD14-1247(p<0.01), but there were not significant difference in eosinophil levels. In atopic asthma, there were no statistical significance in both serum total-Ig E and eosinophil levels among genotypes of CD14 gene SNPs. In addition, allele A frequency of the CD14-1247 had significant difference between atopic asthma and non-atopic asthma(p=0.025).Conclusions Specific allergen of bronchial asthma in coastal area of Shandong Province is similar with other regions but also with the specificity of area location. There was statistical association between serum total-Ig E level and CD14 gene promoter SNPs in non-atopic asthma, but no significant association in atopic asthma. Eosinophil level was not associated statistically with CD14 gene promoter SNPs in both atopic asthma and non-atopic asthma.
