Clinical Study on the Relationship of Plasma NO and ET to TCM Syndromes and to Blood Stasis Syndrome in Chronic GlomerulonephritisMajor:Internal Medicine of TCM Postgraduate:Zheng Hongyi Tutor:Prof. Ruan ShiweiABSTRACTBackground: The disturbance releasing of endothelin (ET) and nitric oxide (NO)plays an important role in occurence and development of chronic glomerulonephritis (CGN)Objectives: To reserch on the changes of plasma NO and ET in CGN with TCM differentiation to explore The relationship of plasma NO and ET to TCM syndromes and to blood stasis syndrome (BSS),which provides index for clinical treatment of CGN.Materials and Methods:123 patients of CGN were selected (61 male,62 female), average age 41.26 ?16. 57(18-65). According to the differentiation standards of TCM, 26 patients were deficiency syndrome of lung and kidney-qi, 37 patients were deficiency syndrome of both qi and yin, 32 patients were deficiency syndrome of spleen and kidney-yang and 28 patients were deficiency of liver and kidney-yin. Among the 123 patients,81 belonged to BSS and 42 belonged to none blood stasis syndrome (NBSS). At thesame time, 40 healthy people were selected as normal control group(21 male, 19 female), average age 42.48 + 15.90(15-63).The plasma ET level was determined by radio immunoassay. The plasma NO level is determined by GRIESS method.Results: (1) The plasma ET level in CGN was significantly higher than that in the control group (P <0. 01). The plasma NO level in CGN wasn't different from that in heath (P>0. 05). (2) The plasma ET level in health was lower than the deficiency syndrome of lung and kidney-qi, lower than the deficiency syndrome of spleen and kideny-yang, lower than the deficiency syndrome of both qi and yin and lower than the deficiency syndrome of liver and kidney-yin. The plasma NO level in deficiency syndrome of lung and kidney-qi was the highest, then the deficiency syndrome of both qi and yin,the health, the deficiency syrdrome of liver and kidney-yin and the deficiency syndrome of spleen and kidney-yang followed in sequence. The plasma ET wasn' t correlated to the plasma NO in the deficiency syndrome of lung and kidney-qi and the deficiency syndrome of both qi and yin (P>0. 05). The plasma ET was negatively correlated to NO in the deficiency syndrome of spleen and kidney-yang and the deficiency syndrome of liver and kidney-yin (P<0.05). (3) The amount of BSS in the deficiency syndrome of liver and kidney-yin and the deficiency syndrome of both qi and yin was more than that in the deficiency syndrome of lung and kidney-qi andthe deficiency syndrome of spleen and kidney-yang (P< 0. 01), The amount of BSS in the deficiency syndrome of liver and kidney-yin wasn' t different from that in the deficiency syndrome of both qi and yin. (4) The plasma ET level in BSS was significantly higher than that in health(P < 0.01), while the plasma NO in BSS and the plasma ET and NO in NBSS weren't different from the health (P>0. 05) ; The plasma ET level in NBSS was significantly lower than that in BSS (P < 0.01); The plasma NO level in NBSS wasn't different from that in BSS(P>0. 05).Conclusion: (l)The disturbance releasing of ET and NO may be related to the occurrence and development of CGN. (2) The increasing of plasma ET can serve as an index to judge the degree of deficiency of kidney-yin. The changes of plasma NO can reflect the degree of renal impairment. (3) The deficiency syndrome of kidney-yin was closely related to the BSS. (4) The plasma ET can serve as a reference index to judge the existance of blood stasis.
