Purposes: To examine the expression of 1-IPV16/18 E6, p53,21WAFl and PCNA in laryngeal carcinoma and investigate the role and mechanism of HPV in laryngeal carcinogenic progression by learning the relationship of HPV 16/18 E6, p53, 21WAF1, PCNA immunoreactivity and clinicopathogical parameters in laryngeal carcinoma. Methods: 30 history confirmed specimens of primary laryngeal carcinoma were subjected to immunohistochemical analyses of HPV 16/18 E6, p53, p21 WAFI and PCNA. 30 cases of Vocal cord polyps and 6 cases of normal laryngeal epidermal tissue were selected as control. Results: 1 The expression of HPV16/18 E6, koilocyte, 21 WAFI PCNA and the relationship with clinicopathological parameters: (1) The positive rate ofHPV16/18 E6, koilocyte, p53, p21 PCNA in laryngeal carcinoma is separately 56.7% (17/30) , 46.7%(14/30), 60% (18/30) , 26.7% (8/30) , 70% (21/30) . In vocal polyps, the positive expression was obtained in 12 cases for koilocyte (40%) and in 16 cases for p21 WAFI(53 3%) and the others are negative. No positive expression is found in normal laryngeal tissue. (2) There is a significant difference between laryngeal carcinoma and control groups in HPV1 6/18 E6, p53 and PCNA expression. Between carcinoma and polyps, no significant difference was found in koilocyte expression, but significant difference was seen between carcinoma and normal tissue. Difference between carcinoma and polyp is significant in 21 WAFI expression, but not significant between carcinoma and normal tissue. (3) The expression of koilocyte and p21 shows a positive correlation with histological differentiation in laryngeal carcinoma. Negative correlation is observed between expression of H1PVI6/18 E6, PCNA and histological differentiation in carcinoma. p53 expression is significantly correlated with the history of smoking. 2 The correlations among HPV16/18 E6, koilocyte, p53, p21 , PCNA expression: (1) The expression of HPVI6/18 E6 shows a positive correlation with koilocyte and PCNA expression and negative relationship with p2l No correlation is obtained between I-IPV1 6/18 E6 and PCNA expression. (2) The expression of p53 shows a positive correlation with PCNA expression and no correlation with p2JWAII expression. (3) There is no correlation between p21 and PCNA. Conclusion: It is an important mechanism that I-IPV 16/18 E6 protein suppresses the expression of p21 1, which disturbs the control of cell cycle. The mutation of 53 gene has no association with FIPV16/1 8 E6 protein expression, but shows a correlation with smoking. The result of this study supports the theory that multi-factor cause the laryngeal carcinoma and provides theoretic basis for the diagnosis and treatment of laryngeal carcinoma.
