Feasibility Studying and Primary Discussion of Clinical Application on Arthrography of Dispersion Method of MR of Knee Abstract Purpose: 1. Feasibility studying on contrast medium injected into vein and dispersing in knee articular cavity and searching the best parameters of the examination. 2. Appling dispersion method of MR to knee arthrography to detect dilacerations of meniscus and discussing the clinical application worthiness. Material and Methods: 1. The first part: Selecting 24 normal knee joints, which are divided into two big group after scanned by routine methods. One is motion group, the other is no motion group. Each big group is divided into three small groups, for each of which MR contrast medium (Gd-DTPA) with three different kinds is injected into the vein. After injection, the no motion group sleep in the examination bed for delayed scanning; the motion group is scanned after 10 minute motion for delayed scanning. After scanning, signals of tissues from the images of plain and enhancement scannings around the articular cavity in cavity are measured. The data collected are classified and managed with the statistics methods. 2. The second part: Examining 24 patient抯 knee joints which are considered to have dilacerations of meniscus. First, these patients are examined by routine methods. And then, they are examined by the best examination methods ( doses of contrast medium, whether with motion or delayed scanning) which are selected from the first part. All the patients examined are approved with the arthroscope or by the operation. Conclusions from the routine scanning, dispersion method of MR of knee arthrography and operation will be compared . The sensitivity and the specificity of plain scan and dispersion method are deduced respectively for evaluating 3 ~mL$~ MR ~ whether the dispersion method is better than that of plain scan, and the image quality and definition of the focus of dispersion method are better than plain scan. In this way, the clinical value of dispersion method can be determined. Result: 1. Arthrography of dispersion method of MR of knee can do obvious enhancement to articular cavity signals and can make significant difference between the signals of articular cavity and the around tissues of the articular cavity. With this method the effect of arthrography can be achieved. The way which adopts 0.2mmollkg of contrast medium and 10 minute motion after injection can obtain the best effect. 2. It can obviously heighlight the signal value of the dilacerations of meniscus of arthrography with dispersion method of MR of knee compared with the plain scan. The sensitivity is improved from 74.2% to 94.3%, and the specificity improved from 88% to 94.3%. Conclusion: Arthrography of dispersion method of MR of knee has a perfect effect in arthrography. It is a new examination method which is simple in operation , with little danger and few complication. It is obviously better in image quality and sensitivity of examination than route examination in detecting dilacerations of meniscus and has vast prospects foreground in detecting other pathological changes of knee joint.
