Objective: Liver transplantation is the last treatment of various advanced liver disease. Acute rejection (AR) is the most common complication and reason for postoperative mortality after liver transplantation. Up to now, diagnosis of AR is based on a combination of clinical signs, liver function tests, and core needle biopsy (CNB) of hepatic graft. It is deficiency of specificity and sensitivity. The purposes of this study were to determine the value of detecting of the gene expression of TNF-a and TNF-~3 in graft, peripheral blood for prediction and diagnosis of AR. Method: we established a rat liver transplantation model without revascularization of the hepatic artery using the cuff technique. Rats undergoing liver transplantation were divided into allogeneic group comprised SD recipients of Wistar livers, and syngeneic group comprised SD recipients of SD livers. SD rats only undergoing laparotomy were served as the control group. The diagnosis of AR in the rat was made on the basis of histopathologic features. Then we used the RT-PCR technique to detect the gene expression of tumor necrosis factor-ce (TNF-c~) and tumor necrosis factor-f3 (TNF-p) in graft, peripheral blood in 1,2,3,5,7 days after liver transplantation. The relationship was made sure between gene expression of TNF-cL and TNF-j3 in graft, peripheral blood in allogeneic group and syngeneic group and AR following liver transplantation in rat by statistics. Results: the gene expression of TNF-c~ and TNF-j3 in graft between the allogeneic group and syngeneic group was not difference (P>O.05) and also in AR group and 2 nonAR group (P>O.05) the gene expression of TNF-ct was not difference (P>O.05) ,but the gene expression of TNF-~ having marked difference (P
