Pregnancy-induced hypertension syndrome (PIH) is the most common complication of pregnancy, a multisystem disorder characterized by hypertension, edma and proteinuria that occur after 20 weeks of pregnancy. It is one of the leading causes of maternal death and a major contributor of maternal and perinatal morbidity. Despite considerable research, the pathogenesis of PIH remains unclear until now. Many researches demonstrate that tumor necrosis factor-alpha (TNFα) and other inflammatory cytokines overproduced by the placenta play a important role in pathogenesis of PIH. Especially, TNFα is intensively related to the pathogenesis of PIH. [Objective] By comparing the various site and strength of TNFαand p55 TNFR expression in placenta of normal pregnant women with PIH, the function of TNFαand p55 TNFR on the pathogenesis of PIH was to be studied.[Methods]Placental tissue was collected at cesarean delivery form 12 normal pregnant women, 15 patients with mild PIH, 11 patients with moderate PIH and 17 patients with severe PIH. TNFαand p55 TNFR expression in placental tissue were detected by using immunohistochemical technique ,and TNFαand p55 TNFR expression in different kind of cells of placenta were also analyzed.[Results] The results show, that TNFαand p55 TNFR were expressed in placental tissue both maternal side and fetal side ,and the staining degree of placental tissue increased following the severity of PIH. The weak immunostaining of TNF-αand p55 TNFR were seen in placental tissue of mild PIH and normal pregnant, and the staining degree has no significant differences between the groups of mild PIH and normal pregnant(P>0.05). The staining degree is higher in patients with moderate PIH than the groups of normal pregnant and mild PIH(P>0.05).The stronger positive expression of TNF-αand p55 TNFR were seen in the placental tissue in severe PIH groups than moderate PIH groups, normal pregnant and mild PIH groups(P>0.05).It is no significant difference that TNF-αand p55 TNFR in the groups of normal pregnant and mild PIH were expressed between maternal side and fetal side of placental tissue(P>0.05).In severe PIH groups and moderate PIH groups ,the staining degree of maternal side of placental tissue is higher than fetal side(P<0.05). TNF-αand p55 TNFR were expressed in decidua cells, endothelial cells, villous stromal cells, cytotrophoblast , and syncytiotrophoblasts .The kind of positive cells of TNF-αand p55 TNFR is no difference between the groups of PIH and normal pregnant. The staining degree of TNF-αand p55 TNFR in the same kind of cells increased following the severity of PIH. In the same group, the staining degree of the different kinds of cells is different, the strongest staining degree of TNF-αwas found in decidua cells, the staining degree became weaken gradually in endothelial cells, villous stromal cells, and cytotrophoblast. The weakest staining degree was found in syncytiotrophoblasts. The strongest staining degree of p55 TNFR was found in decidua cells and endothelial cells, and the weakest staining degree was also found in syncytiotrophoblasts.[Conclusions]A little amount TNF-α and p55 TNFR expressed in normal pregnant women may participate to regulate the normal gestation. The expression of TNF-αand p55 TNFR increased in moderate and severe PIH groups, the staining degree is positive correlation with the severity of PIH. In moderate PIH and severe PIH groups ,The staining degree of TNF-αand p55 TNFR in maternal side of placental tissue is stronger than in fetal side. In the groups of normal pregnant and PIH, there is no significant difference in the kind of positive cells of TNF-αand p55 TNFR. The staining degree of the same kinds of cells is positive correlation with the severity of PIH. the strongest staining degree of TNF-αwas found in decidua cells, the staining degree became weaken gradually in endothelial cells, villous stromal cells, and cytotrophoblast.
