Despite the dramatic improvement in treatment success achieved during the past several decades, approximately 50-80%of patients with leukemia still suffer a recurrence. Recurrence has become essential obstacle of disease-free survival. Therefore, the study of understanding pathogenesis of acute leukemia (AL) and to lower the rate of recurrence and treatment failure remains an important issue. Clinical trials have demonstrated that some cytokines play an important role in the pathogenesis of AL. Among the heterogenic group of cytokines, the tumor necrosis factor (TNF) regulates two opposing activities: a destructive and protective effect on AL cells. The aim of the study was to examine the relationship between serum levels of TNFα/β,sTNF-Rs (soluble tumor necrosis factor receptors) sTNF-R1/R2 and the gene expression of two types of receptor for TNF(TNF-Rs), TNF-R1/R2. and to explore the correlation between TNF and its receptors and the clinical course of AL .To find out the index at molecule level for assessing the effectiveness of therapy and monitoring early relapse. TNFα/βand sTNF-R1/R2 levels in sera from patients with AL were measured by enzyme-linked immunosorbent assay (ELISA). Expression of TNF-R1/R2 mRNAs in bone marrow was analysed by Reverse Transcription-Polymerase Chain Reaction (RT-PCR). TNFα/βand sTNF-R1/R2 levels in sera from 44 patients with AL were measured and expression of TNF-R1/R2 mRNAs in bone marrow from 34 patients with AL was analysed. to examine the relationship between serum levels of TNFα/β,sTNF-R1/R2,the gene expression of TNF-R1/R2 and disease-related prognostic factors (archaeocyte, megakaryocyte, leukocyte, erythocyte, blood platelet, and hemoglobin). Conclusions: 1. The study established standard curves in order to examine serum levels of TNFα/β,sTNF-R1/R2 with AL during initial treatment,remission and recurrence. 2. The study demonstrated that expression of TNF-R1/R2 mRNAs in bone marrow cells with AL is analysed by RT-PCR. 3. The densitometric analysis of RT-PCR products was performed by calculating the relative intensity (IOD), IOD=OD×Pix,that is, the value of the intensity of bands for TNF-R1/ R2 divided by the intensity for β-actin in each sample.4.The elevation of TNFα/β,sTNF-R1/R2 in the sera and strong expression of TNF-R1/R2 in bone marrow of patients with AL during initial treatment,remission and recurrence. Thus, TNFα/β,sTNF-R1/R2 and TNF-R1, leading to proliferation of leukemia cells in AL. 5. Both TNFαand sTNF-R1/R2concentrations in sera were significantly higher in initial treatment for leukocyte count>100×109/L. 36.34±21.64pg/ml, 14.63±1.99ng/ml, 5.08±1.39ng/ml respectively,and suffer a recurrence furthermore. 6.
