Background and Objective: The coronary Doppler Guidewire is the most advantageous tool in measurement of coronary flow velocity and evaluation of microvascular pathology. It can be used to discriminate the cause of chest pain in patients with normal coronary angiogram. In order to decrease the measurement error, someone consider using nitroglycerin before adenosine. But there has different view. The purpose of this study is to evaluate the effects of nitroglycerin and adenosine on coronary flow velocity using intracoronary Doppler flow measurement in normal angiographic coronary arteries.Methods: In 26 normal angiographic coronary arteries of 10 patients, coronary artery flow velocity were measured before and after intracoronary nitroglycerin or adenosine administration, the coronary artery flow velocity reserve (CFVR) were calculated.Results: Both nitroglycerin and adenosine accelerate the coronary artery average peak velocity (APV(cm/s) from 18.5 + 4.3 to 29.0 + 5.4, ;XO. 001 and from 18.1+4.0 to 44.5 + 12.5, p<0. 001 .respectively). The base APV has no significant difference before using of nitroglycerin and adenosine , but thehyperemia APV(cm/s) has significant difference (44. 5?2. 5vs.29. 0?. 4, p<0. 001). The CFVR measured with adenosine before andafter nitroglycerin bolus inject has no difference (2. 53?. 66vs. 2.51+0.62, /F0.671) .Conclusion: l.Both nitroglycerin and adenosine induce coronaryartery hyperemia response and accelerate coronary artery flowvelocity, but adenosine has more powerful effect thannitroglycerin.2. The nitroglycerin has no effect on the measurement of CFVR byusing of adenosine.
