Objectives. The aim of this study was to evaluate long-term results and to assess prognostic factors which have an impact on overall survival in patients with advanced epithelial ovarian carcinoma.Methods. From Jan 1990 to Jan 1995, 208 patients with stages II~ IV epithelial ovarian carcinoma admitted to our hospital for primary treatment, were eligible for retrospective analysis . All operations were performed by senior surgeons. All patients were given systemic and/or intraabdominal combination chemotherapy. Histologic sections were reviewed by the same pathologist. Statistical methods included Kaplan-Meier survival curves, log-rank test, and multivariate analysis.Results. The 5-year survival rates were 52, 49, 34, 22, and 3% for patients with stage II, Ilia, Illb, IIIc and IV disease, respectively. Age, FKiO stage, histologic type , exten of surgery, and the number of courses of chemotherapy were significant prognostic indicators in uni variate analysis. Multivariate analysis that the risk of mortality according to FIGO stage was 0. 088, 95% CI [0. 031-0. 249], P< 0. 001 for FIGO II;0.128, 95%Cl[0. 082-0. 358], P=0. 001 for FIGO IIIa; 0.290, 95% CI [0.159-0.535], P = 0.001 for FIGO Illb ; 0.594,95% CI[0.372-0.947], P=0. 010 for FIGO IIIc;in comparison with FIGO IV. Patients with a serous epithlial carcinoma had a 1. 6-fold higher risk of mortality than patients with other histologic types, RR=0.638 95% CI[0.454-0. 897] P=0.010. The prognosis of advanced epithelial ovarian carcinoma following cytoreductive surgery is influenced by residual tumor, and the number of courses of chemotherapy.
