Objective To investigate the alteration and clinical significance of the plasma factors associated with apoptosis soluble Fas and Fas ligand(sFas,sFas-L) in patients with chronic congestive heart failure caused by hypertension .Methods The study included 21 patients with chronic congestive heart failure caused by hypertension and 19 patients simply with hypertension, 24 age-and gender-matched normal control subjects, in the heart failure gronp,the coronary heart disease was ruled out by coronary artery radiograph. Plasma levels of soluble Fas and Fas ligand were measured by enzyme linked immunosorbent assays. Left ventricular ejection fraction (EF) was also measured in order to investigate the relationship of the plasma levels of soluble Fas,Fas ligand and EF.Results The plasma soluble Fas and Fas ligand levels were 44. 85 + 19. 63ng/ml and 805. 93 + 626. 23pg/ml which were significantly higher in patients with heart failure than in normal subjects(25. 18+12. 37ng/ml,491. 18?7. 87pg/ml)and in patients simply with hypertension(27. 68 + 11. 08ng/ml,473. 23 + 96. 68pg/ml) (P<0. Oland P< 0. 05). There was no significant difference in sFas,sFas-L levels between normal subjects and patients simply with hypertension (P>0. 01). In patients with congestive heart failure the leves of solube Fas and Fas ligand were in negative relevance with the left ventricular ejection (EF) (r=-0.4748 and r=-0. 7014,P<0. 05 and P<0.01).Conclusions We found elevated levels of plasma soluble Fas and Fas ligand in patient with congestive heart failure caused by hypertension. The incease in soluble Fas and Fas ligand may play an important role in the pathophysiologic mechanisms of chronic congestive heart failure.
