Objective To evaluate the dynamic changes of plasma tumor necrosis factor alpha (TNF-a). interleukin-6 (IL-6), interleukin-6 (IL-8) and nitric oxide(NO) levels in neonatal with hypoxic-ischemic encephalopathy (HIE), and explore the effects and the interface of them on HIE.Methods plasma IL-6> IL-8, TNF-a and NO levels were measured in 46 neonates with HIE (18 cases of mild. 17 cases of moderate and 11 cases of severe status) and 18 normal term neonates by enzyme-linked immunosorbent assay and nitrate reductase method at time of admission.the first day .the seventh day and the tenth day after birth.Results ( 1 )Plasma NO level in infants with mild HIE was significantly higher than that in control at admission time(in the control in two hours of birth) ( P<0.0 1 ) and resumed to normal at the first day after birth(p>0.01). Plasma IL-8, IL-6, TNF- a level in infants with mild HIE was no significantly differences compared with the control at any time(p>0.01). Plasma NO, IL-6, IL-8.TNF- a level with moderate HIE was significantly higher than that of at admission time ( P<0.01 ) , and peaked at the first day after birth .then decreased gradually and resumed at tenth day after birth. Plasma TNF- a .IL-6.IL-8 and NO levels with severe HIE were significantly higher than that in control at admission time. (P<0.01 ) ,and peaked at the first day after birth then lasted until the third day after birth.Plasma NO. TNF- a ,IL-6 levels with severe HIE resumed gradually at the tenth day after birth(p>0.01). While plasma IL-6 level with severe HIE was sinificantly decreased at the tenth day after birth (PO.01 ) . but was still sinificantly higer than that in control (PO.01 ) .(2).Plasma NO IL-6. IL-8. TNF- a levels with severe HIE were sinificantly higher than that inI!3Abstractmoderate HIE at admission time. and at the first day after birth (PO.01) .Plasma NOJL-6,IL-8.TNF-u levels with severe HIE were sinificantly higher than that in moderate HIE at the third day after birth too (PO.01 ) .(3) Plasma NO level was were sinificantly relevant to Plasma IL-6.IL-8.TNF- a levels ( PO.05 ) Plasma TNF-a level was sinificantly paralled with to Plasma IL-6JL-8 levels ( PO.05 ) Plasma IL-6 level was sinificantly related to Plasma IL-8 levels ( PO.05 ) .Conclusion The present data suggest that the changes of plasma NO,IL-8,IL-6, TNF- a levels were correlated with severity of HIE. It is also indicated that IL-6 might play a role in the pathogenesis of neonatal HIE. These cytokines. with other inflammatory media, formed a network and played an important role in the earlier pathologic process of HIE.The present data have a value for diagnosis,prognosis and treatment of neonatal HIE.
