Objective To study the changes of molecular markers of hemostatic system in childrens with Henoch-Schonlein purpura(HSP)and to evaluate its mechanisms in the pathogenesis of HSP and their clinical significance.Medthods 41 HSP patients were divided into the group of Henoch-Schonlein purpura nephritis(HSPN) and the group of non-HSPN,the control group consisted of 20 health children. The plasma levels of von Willebrand fact(vWF), granul emembrane protein-140 (GMP-140),proteinC(PC) and D-Dimer(D-D) were measured by the method of double antibodies ELISA , developing substrate method was used to examine the activity of tissue-type plasminogen activitor(t-PA) and plasminogen activitor inhibitor-l(PAI-l).Results In the prime of HSP, the plasma levels of PAI-1 activity, vWF, GMP-140 and D-D were significantly higher than that of the control group (P<0.01). Furthermore, the levels of these molecular markers were higher in HSPN than that in non- HSPN. On the other hand , the levels of t-PA activity and PC were decreased significantly than that of the control(P<0.01). After HSP was controlled,the levels of PAI-1 activity, vWF,GMP-140 and D-D were significantly decreased than that of primary, whereas the levels of t-PA activity and PC were increased significantly(P<0.01). But in HSPN,the levels of PAI-1 activity, vWF and D-D were also higher than that of the control, while t-PA activity was slightly lower than that of the control, the levels of these molecular markers were gettingnormal in non- HSPN.Conclusion These findings indicate that there are vascular endothelial damage, elevated platelet activation and prethrombotic state in HSP.The levels of these molecular markers were changed significantly in HSPN.The abnormal of hemostasis may play an important role in the genesis and development of HSP,especially Of HSPN.It is helpful to diagnose prethrombotic state in childrens with HSP by examining these markers,and to predict the prognosis of HSP.It may be useful for guiding treatment.
