Objective:To define MRI criferia for the diagnosis of the invasive pituitary adenoma. Methods:The MR images obtained in 103 patients were reviewed retrospectively. The golden standard for invasion were the surgical findings. A X2 test was performed,and the sensitivety,specificity,positive predictive value (PPV),and negative predictive value (NPV) of each group of MR imaging signs were computed. Results: The MR imaging signs for the diagnosis of the invasive pituitary adenoma:The turmor infiltrated into the sphenoid sinus (PPV 100%) ;The percentage of encasement of the internal carotid artery (ICA) by turmor was 5/8 or greater (PPV 97%) ;The line joining the lateral wall of the intracavernous and supracavernous ICA was passsed by the tuomor (PPV 100%) ;All compartments of the cavernous sinous were not depicted (PPV 100%). (2) The MR imaging signs for exclusion of the diagnosis of the invasive pituitary adenoma :The sellar floor bone was not eroded and the morphosis of the sellar floor was not changed (NPV 100%) ;The percentage of encasement of the internal carotid artery(ICA) by turmor was less than 2/8 (NPV 100%);The line joining the medial wall of the intracavernous and supracavernous ICA was not passsed by thebetween theturmor and the medial wall of the cavernous sinous (NPV 100%0. (3) The MR imaging signs for highly probale diagnosis of the invasive pituitary adenoma:The percentage of encasement of the internal carotid artery (ICA) by turmor was 4/8 or greater (PPV 95%). Conclusions:The above-mentioned criteria may be of assistance to the diagnosis of the invasive pituitary adenoma.
