Purpose To evaluate the clinical effects and the influent factors of cataract phacoemulsification, by following up and observing the long-term and short-term clinical effects of patients with cataract phacoemulsification (Phaco) and intraocular lens (IOL) implantation. Methods After topical anesthesia, patients with cataract were operated on phacoemulsification by the same surgeon. Analyze the visual acuity, complications and their influent factors of 221 cases (297 eyes) and 167 cases (237eyes) for short term and long term, respectively. At the same time, observe and evaluate the influences of anterior continuous curvilinear capsularhexis (CCC) and intraocular lens (IOL) on posterior capsule opacification (PCO).Results Corneal edema after surgery for short time is the main complication (13.13%, 39 eyes), of the 37 eyes (94.87%) have more harder cores (IV-V). 89.89% of eyes after three-day surgery , the corrected visual acuities can reach3≥0.3; the rate of PCX) which can impair the visual acuity is 28.27% after 2 years. Anterior free CCC edge partly off the optic can cause worse PCO than the free edge on the optic (PO.0005). In the group of CCC on the optics completely, the IOL with sharp-edge can better prevent the PCO than the round-edge IOL (P<0.0005). And lOLs with the same sharp-edge and different materials (acrylic and silicone) may not influence the PCO significantly (P>0.05).Conclusions Under the topical anesthesia, cataract phacoemulsification is safe, highly effective and can make the patients recover the visual acuity quickly and get good effects. The main complications after surgery are edema and PCO for short term and long term, respectively. The anterior CCC and the design of optic are the importantly influent factors for PCO. By performing the completely medium, suitable-size anterior CCC and choosing the sharp-edge IOL can reduce and prevent the PCO effectively.
