Clinical Diagnosis And Treatment Of Intraocular Lens Opacification After Cataract Surgery (Six Case Reports)

ObjectiveTo investigate the clinical manifestation. diagnosis and treatment of IOL opacification, and analyze the research approach,possible mechanism. diagnosis standard and treatment way of its onset. To provide the foundations for prevention. early diagnosis and treatment.MethodsCases of IOL opacification and who taken IOL exchange in the inpatients of 2nd affiliated hospital of Zhejiang University from January 2002 to August 2009 were studied; and did clinical pathology research to some IOLs by slit-lamp,scanning electronic microscope (SEM) observation and X ray diffraction. We searched the PubMed database of the National Library of Medicine online form 1980 to 2010 using all related terms (cataract. IOL opacification,phacoemulsification. IOL implantation,hydroxyapatite. hydrogel. acrylic,silicone. IOL exchange),and comprehensively analyzed the mechanism. diagnosis. research approach and treatment.ResultsThere were 6 cases from January 2002 to August 2009,3 male.3 female, average age was 73.3 (68-80),5 in right eye and 1 in left eye, average time form IOL implantation to opacification was 68 (12-122) months.3 had eye disease and all 6 had systemic disease,1 was misdiagnosised to posterior capsular opacification and taken YAG laser treatment. All patients taken IOL exchange, average pre-operation and post-operation visual acuity was 0.21±0.09 and 0.50±0.21, Paired-samples t-Test, P=0.013 < 0.05, the difference was significant. Opacified IOL were hydrogel and manufactory date was from 2000 to 2002. Slit-lamp observation of one IOL showed haze opacification in post surface, SEM showed opacification was loose and mussy crystalloid deposits on the optical surface, calcium dihydrogen phosphate monohydrate was detected by X ray diffraction.Conclusion(1) IOL opacification is a kind of rare disease and mainly happened in hydrogel IOL and so on, it is essentially for the deposits, which was mainly calcium-phosphorus compound, on IOL.(2)IOL opacification is easily misdiagnosised to PCO and other diseases,and be taken unreasonable treatment.(3) IOL exchange is an effective treatment.
